I just explained to you why he would have given him antibiotic. I understand the mechanism of antibiotic resistance, and the need for responsible prescribing. I definitely dont need you to try to educate me on it, thanks.

As i said, the skin flora (eg staph and strep) are what will commonly cause a superimposed skin infection after a potential insect bite. They are both gram positive. All of the antibiotics i listed above cover both. Keflex has good coverage as well, essentially everything youd want to cover for except MRSA.

You dont culture every single little wound that comes into the ER. We get all caught up in "the appropriate diagnosis" (ie. putting a fancy name to something) when it is still treated the same regardless of what the fancy bacteria name comes back as. Its an absolute waste if money when 90+% of skin infections are caused by the same couple of pathogens that are all treated with the same few antibiotics. As i said above, you engage in shared decision making with the patient. Lay out the positives of starting the abx and the negatives of them, then come to a decision together based on what the patient feels is best when they understand both sides. State the fact you're not 100% sure its actually a bacterial infection but it does seem to be showing signs its going that way. If you think the patient is responsible enough this might be a good time for a "wait and see" prescription.

We weren't in the room with DC and the ER doc. You're just up in arms over something that you really only have about 1/3 of the story on, the part that he has this wound and you disagree with someone who looks at skin infections several times a day for their job. You want to treat people like DC? Move to the US, go back to school, get a degree and change the world man.

The diagnose should be : Possible bacterial infection. Cause unknown -and not "spider bite" in absence of a spider.

Sorry to have hit your clitoris, apparently you are a bit pissed: "I definitely dont need you to try to educate me on it, thanks."
I stated my opinion. If it in fact were MRSA with multiple resistance to AB's (what can not be ruled out!) DC's case could have gotten complicated without a proper diagnose. Its fact and common sense. Now, I understand that may not be needed in most cases, if the bacteria is resistant to the first AB, Cowboy will show up again, with a bigger hole in his armpit and we just prescribe another one. Expensive to cultivate a bacteria and to see how it reacts to an AB?
The money the current health system charges for even easy procedures is an assault on people. Its an international phenomenon with few exceptions.

Just for info:
Can spiders be carriers of human pathogens? Can they provoke infection through a break in the skin? A team of scientists has data-mined the history of publications on spider envenomations to conclude that the evidence for spider-vectored infection is scanty. Although spider bite may be an attractive and tenable causative agent of a bacterial infection, the data show this is highly improbable, says the study's lead author.

HGP, it was the VA Hospital where I also work. Knowing that I work there too, they did what they call Fast Tracking. She looked at, went with the most quickest diagnoses and said that it did look most like Necrotizing Fasciitis from a spider bite. So they prescribed an antibiotic and an Anti-inflammatory. It has since cleared up, but just hasnt healed fully yet. But since I just took the last pill it is looking good.

If it really was Necrotizing fasciitis (rare!) /and or a spider bite, we will never know. You are almost healed and it doesn't matter.

Ah, I agree with @Wobmarvel, flex the biceps when posing to show your guns. I know, posing may not be your strength (mine neither), I bet @BEAST73 can help you with that.

Weird comment this but I'm not convinced your flexing your biceps in these pics, your hand appears open. Same with other double biceps pics. Clench your fist and they will look bigger. Some people simply can't flex that well (Paul dillette anyone). I get that in the grand scheme of things who gives a f*CK but if your posting progress pics you want to make them as impressive as possible.
Thanks @Wobmarvel. I do need to get better at taking pics

No probs, was worried you would think I was trying to start sh1t but I see a lot of this. I'm clearly no expert but if you do that exercise where you use a cable crossover settup but hold a handle on each hand and do standing bicep curls as if your doing the pose, the cables will pull your hands away and you will have to flex. I bet it makes a world of difference to how your arms appear, that or get your wife to try and pull your hand away from your body as you flex so that you get a feel for the flex.

Definitely getting leaner. Your traps are by far your most developed muscles. Did you carry heavy weight long distance with straight arms when on the military by any chance? They look like "farmers walk" traps.
I carried a rucksack, and heavy machine guns

Traps definitely beefing up nice. Gyno isn’t going away without surgery - it’s just what it is. Surgery ain’t that bad if he ever springs for it, ‘specially not for a tough SOB like DC. Just takes a while to heal & pretty achey for a bit. Although me having hematomas probably didn’t speed that up any

I seen it first when I was really fat
I’ve had gyno since puberty; I was 200lbs when I was 10 and started getting hair on my nuts - that’s a LOT of aromatase enzyme stored in all that bodyfat when all that test came blasting through!

Hormone use just made it worse, but looking at old pics I was always screwed. Oh well, my dad is 69 with hair and I don’t shed on any compounds, so I got that going for me at least.

Ralox will at a minimum make it shrink while on it, and may permanently reduce it.

Honestly my surgeon wasn’t very good, and it was still the best 6 grand I ever spent. Things will never be perfect, but it made a great difference.
