Kleen - Project 50: Under Construction

This is something I try to keep an eye on and limit my insulin spikes to one or two a day even when not eating "properly". Lots of Type 2 Diabetes in my family.

I have been staying on top of cardio / walking to burn fat as well as hitting my weight training. I did not end up doing any lower body the other day after my afternoon cardio Wednesday. I started warming up but my right hip flexor was just really tight and uncomfortable. It just didn't want to loosen up. It was still feeling a bit tight yesterday, so I decided to give it a little time to recover from whatever had it so inflamed. I walked outside for an hour at lunch yesterday then did some upper body yesterday.

Still in a feeling out stage on the shoulder so I am slowly adding in more volume and frequency. Yesterday I did a horizontal Push / Pull session. Kept the volume and weights low. Worked the muscle decently with 2 exercises and called it a night.

Training from yesterday
Lunch Session - 60 minutes brisk walk

Evening Session - Horizontal Push Pull - Short Rest periods
Elliptical = 6 minutes increasing intensity then RC exercises

IronTech Iso-lateral Low Incline Chest press - 5 work sets - 25-8 reps
done with intention

Recline Ring Rows - 4 sets AMRAP - 22-7 reps - it is funny how quickly these become hard...

I know this doesn't look like a lot but I have slight DOMS in my pecs and honestly at this point it doesn't take many sets for me to really tax the muscle with my mind muscle connection. Plus I am still unsure how much volume I can push before causing an issue with my shoulder and need to figure that out. I don't see the PT until next Thursday so kind of flying blind and after how stiff my shoulder got after the last higher volume upper body session I am a bit leery to jump back. I am adding things back in slowly to see what I can get away with without any deleterious effects.

About to go do some cardio and then tomorrow will be a leg workout. Planning to push them a bit with a higher volume workout at the gym.

For whatever reason my digestive system went on strike and that steak sat in my gut causing gas, nausea and hiccups. It wouldn't move, by the morning I was more full and distended. Called in to work from being exhausted and sick all night. Ended up laying on the couch miserable all day and coming nowhere near my caloric needs. All I was able to eat was dinner. I feel fine now but something really messed with my motility for everything to just be stuck in my stomach not moving for almost 24 hours. I was finally able to eat dinner but it was a very light meal. Today I woke up feeling great, no issues at all. Had a good movement which I had not had Sunday or Monday so that was all a part of the lack of motility. Very happy to be feeling better and able to eat.

Sunday - Waking bodyweight 210.6 down from a high of 214.4 2 weeks ago when I started paying more attention to diet and cardio.

Leg Adductors - worked up to 170, 200, 220, 220

Leg Abductors - worked up to 190, 220, 230, 240

That’s my downfall whenever I’ve been cutting calories for awhile and decide on a cheat meal. I usually eat a big old steak with baked potato or Italian food, and inevitably pay the price. Bad part is my wife warns me every time, but between my lust for food, at that moment, and my hard head I usually mess up and pay the price. Thankfully for you it only lasted a day.
I have learned to take Prilosec and Gas X before cheat meals.😂

Oh I wish it was that, the ribeye was only a 20 oz, and me filling up at the half point was more due to my digestion already being off for some reason. The original plan was to crush some marble slab after for dessert but I was not in any place to eat more. My son was full and decided not to go so we didn't but I wouldn't have had any if we did. I definitely intended to enjoy my lust for food on that one but my stomach put on the brakes well before I got to crush anything major. Heck I even left fries on my plate. 😲😲😲

I definitely know the feeling you are talking about. When I ran the Ultimate Diet 2.0 for a while I would feel that way every weekend. It got to a point I was like Oh hell no, I am not eating that much food trying to get all those carbs in. It was making me miserable for a few days after the weekly refeed. It's actually funny you mention the wife warning you, and taking preventative medications. If I do have a really big meal where digestion is likely to be a problem my wife just gives me one of her dexalant caps either while eating or right after. LOL

For some reason every once in a while my food will get in my stomach and just sit there like a brick. It won't digest and go into the intestines and just kicks up gas, and often heartburn. This will actually happen even if I am eating lower calorie foods or not.

AMEN!!!

Going to do some upper body at lunch today. Shoulder is a bit tight and sore in a few areas. Starting to think I may need surgery after all. Going to talk to the PT tomorrow and see if they think we should waste time with this or schedule surgery and do this after. Just seems I shouldn't already be feeling some of this stiffness come back so soon and other parts have had no improvement at all. I could be wrong but am not real confident that doing RC exercises is going to do anything for the pain from the labrum tear.

Speaking of recovery and pains, @Marne40 is being very kind and sending me some HELIOS, and RELAX to test out on my shoulder while it is recovering. So I will be mentioning it in here as far as what I am getting out of the products. Big thanks to him to do this out of the kindness of his heart!
 
Being on Dbol specifically seems to do exactly what you experienced, which is probably why it gives me heartburn so much faster than anything. For some reason, it’s like I lose all gut motility. My belly basketballs, I lose any semblance of hunger/appetite, sudden gas and awful heartburn.

It’s a nasty experience!

Glad it’s done, and looking forward to seeing how the Apex stuff treats you. Everything I’ve used from them is a winner.

Got not so great news from the PT today. She told me, "I am surprised to see you here with everything wrong with your shoulder. This should have been a surgery right off the bat. I am not going to be able to do anything to help remove any of your SLAP tear pain. It is a tear that your bone is rubbing into and I can't make that stop. I can strengthen the muscles he asked me to but this is not going to take away your pain. You will need a surgery for that."

She told me she was going to give me the appropriate exercises to do 3-4 times a week but was not going to waste my PTO on coming down to do them in front of her since I was familiar with working out. Schedule me to come back in 2 weeks for a follow up but in the meantime she will call my Dr and tell him her findings and my concerns. Told me to leave and call him and tell him how bad I am hurting again already and that I want it fixed and that she said she would not be able to fix that. She said hopefully in that 2 weeks i will have been able to get a new plan of action set in place. If not then we would finish the requested PT to justify the surgery to the insurance.

I was also told do not do any RC exercises as they would exacerbate the issue, and to avoid curling since it was causing pain in my shoulder as well.

Yesterday's lift session = Trying to find things I could do with upper body comfortably.

Elliptical 6 minutes @ fast pace
Lots of arm movements with 5lb DB, and 10lb DB trying to get blood into shoulder

DB Arnolds - 10lbs x a lot, 15 x a lot, 25x 15ish, 30 x 15ish, 35 x 11,
shoulder got uncomfortable even with this light weight so I stopped well before the weights became challenging.
Lat Pull Downs - 50 x 20ish x 3 sets

Dip Attachment - 70x 20ish, 95x20ish, 120x RPE9 x 4 sets - no count on work sets
, went by if I could do another rep correctly while using as much intention and creating as much tension as previous reps.
NG Pull ups - 5, 5, 7, 5, 4 - stopped at 4 because I felt a little friction on my labrum and didn't want to risk anything.

I tried some light standing curls but it was hurting my biceps tendon. I can only do curls with my arms in front of me like on a preacher curl. If my arm is down at my side it hurts.

Dips Attachment Notes - I focused on positioning myself so that a lot of the focus was on triceps then created a ton of tension in my chest by trying to squeeze the dip handles together while pressing down. At the same time using my biceps to create extra tension on my triceps so that pecs and triceps were getting hammered while my joints were not getting challenged at all or very minimally. Was actually pretty awesome and even once healed I will use this method for major contraction and tension in the pecs and triceps without killing my shoulders. It also worked my abs pretty hard and produced a good biceps pump from my biceps resisting my triceps on the way down.

Hate to here the non-surgical plan isn’t working out, but at least you have somewhat of an answer and can prepare for the probable surgery. I hate it when insurance companies make you jump through hoops because of their “protocol”. Especially when the Doctor is the expert and could possibly schedule surgery quicker to get you some relief. Insurance Companies are far too powerful and many times slow down the healing process with their crap.

Absolutely and to be sure, I am not going anywhere! Last time I got so frustrated with being injured in both my shoulder and leg and took off I lost a lot of ground that was impossible to get back while still in "limp mode". This time I am staying with it and just changing focus to leaning up. I am going to go down to 195 at the minimum and see how I look there. May just keep on going after that. Your progress makes it easy to want to do it!

Thanks and Agreed! The difference from my first surgery scheduling was night and day. I went in once, did the assessment, he scheduled an mri for me and I had surgery 2 days after he got the results.

Yeah they apologized for being so blunt. I told them "No need for an apology. Thank you for being up front and not wasting my time. I prefer that."

On the dips mine I am doing them standing on the floor with a lever set up. Think like a hammer strength but standing. So it made it easier to really squeeze the pecs. If doing body weight this would be way more challenging and harder to get as much intention or definitely cause you to drop a lot of reps. However it is pretty great contractions.

I decided to go torture chamber mode when I got home yesterday did cardio in the hot garage. I haven't been sweating a lot during cardio even when pushing it, but I sure did yesterday. I sweat so much even my shirt was dripping.

Elliptical - 500 calories
Weighted Crunches - 50lbs x 5 sets AMRAP
- by the last set I was only able to get 8 reps - I was really squeezing the crap out of my abs and found the groove to get the best contraction. I learned to like this part of the Iron Tech gym a lot more by finding the right position for me.

Nutrition has been on point, I was 209.4 yesterday morning when I weighed in, down another pound since my last weigh in. I am going to start weighing daily to help keep me accountable. I can just do a weekly average to track actual progress rather than fluctuations. I definitely need to get used to tracking this stuff again.

Daily Tracking is a big thing for me, it helps me make small changes before things get out of hand.

Yep, daily tracking is so important. I think that’s one of the reasons so many people fail on so many different eating programs, they don’t track their calories. So they have no real clue where they really are and therefore can’t make proper adjustments. A good eating program is only as good as the accuracy of the calories and micros you truly are consuming.

Facts! My progress is leaps and bounds above when I track. To make this cut work I am going to have to prep my own food so I can track easily and just let the family eat whatever they decide to for their meals.

Agreed, a plan doesn't matter if not executed. Heading to the store for some stuff to prep tomorrow. Going to roast a bunch of veggies, and potatoes. Make some turkey chili, or maybe another "muscle casserole". Then cook a bunch of chicken, turkey, and beef. Make sure I have everything I need.

I went to do a higher rep back down set and my knee kind of waivered so I stopped the set.

Started to do leg press and my knee just felt off. I decided it was best to just call it a day. That was a lot of build up sets on squats and more than enough to produce some stimulation. I will get more volume in the gym. Also 310 is new progress during my recovery. I feel I could have gone 315 maybe even 320 but the last rep was hard enough I knew I would not get 5 if I went up again.

I decided I am going to add 24Hr back into the gym rotation since they are doing 10 a month for 1 club only and went back being open 24/7. Sometimes on weekends I really like to go up later and have the place to myself. Plus having a hot tub available is a definite bonus.
 
Muscle casserole, recent squat PRs, and access to a hot tub again - what else do you really want in life, you know what I’m sayin’??

That is one of the home upgrades we are going to get for the house! The wife has been wanting one for years. We are going to close off a section of the back yard with some more privacy fence right behind the master bedroom. Will be one of those kids will have to ask permission to use kind of things.

7/24/22 - Cardio & Abs

Recumbent Bike - 45 minutes 405 calories
Rotary Twist Machine - 50x15 3 sets each side
V Crunch Machine - 15 x 3 sets done 5x1 arm, 5xother arm 5 time both arms, rest and repeat for 3 sets
Crunch Bench 2 sets to burn out.

Sauna 20 minutes just to enjoy the heat and sweating.


Weight is 210.4 today, had a big carb day Saturday after my squat session so burning off extra carbs.

Nice to know I have all my meals ready for today. Will be breaking my fast with some 93/7 turkey taco meat, fat free refried beans, pico, salsa, and a kiwi. Definitely ready for that meal but have a couple hours to go before it is time for that.
 
My wife was the one that wanted a hot tub years ago when we got it, but I bet she only gets in it a couple times a years, I on the other hand am in it 2-3 times a week. When the grand kids come over they always bring there swim suits so they can get in. Just something to remember, if you get your Doctor or Rehab Person to write you a script for the hot tub it is tax deductible ! :ROFLMAO:

Oh wow, that is a great point, I am sure I can get my doc to do that! Yeah the kids will be able to get into this one too. Just need permission. Want it to be relatively private otherwise for the wife and I to just sit and chill with no disturbances. Plus having another fence there will keep my dog from trying to jump or chew the other fence up. The neighbors moved out and their kids moved in with 2 dogs that steadily bark at the dogs, and 2 young kids who think it is funny to beat on the fence and make our dogs bark. So will be a nice buffer from that.

Pretty Frustrated right now... Made an appointment with my ortho for tomorrow. He has a nurse call to tell me he can't do surgery for 3 months after my injection... I am fuggin hot. I told him I did not want to do this if it was going to slow me down in anyway getting things fixed. If I am stuck in pain over this for 3 months because he didn't clarify with me I would have to wait 3 months I am going to be irate. Had he told me that I would have gone to get a 2nd opinion elsewhere and found a Dr willing to fix it up front. I told him do not have me spinning my wheels here. This is just so frustrating I could headbutt a Nun!

Now I am looking into PRP which he probably should have offered me first. See if maybe he does that or can give me a referral to someone who will. Also if I have to wait I am definitely going to run BPC-157, and maybe even see about picking up enough GH to run 2IU a day for a while and running a cycle of collagen producing gear. What I am not willing to do is stop working out, or let my entire left side get weak as hell while waiting on this. So annoyed right now...

Oh yeah I did 35 minutes on the treadmill, and my physical therapy exercises during lunch.

Yeah, just frustrated at this point. I can't believe he didn't tell me ahead of time if he gave me a shot and it didn't help I would be stuck out for 3 months. I have been reading up on PuMed and it looks like they are saying there is no obvious benefit to waiting longer than 4 weeks.

Abstract
Background: Corticosteroid injections administered within 3 months before total knee arthroplasty (TKA) have been linked to increased risk of postoperative infection. However, it would be beneficial to further delineate the timing of injections to determine whether a narrower window exists for safe administration of corticosteroid injections. The purposes of our study were to (1) determine whether there were a different time frame between corticosteroid injection and primary TKA that increased infection risk and (2) determine risk factors associated with infection after TKA.
Methods: TKA patients were identified from a national database from 2007 to 2017 and stratified based on their history of corticosteroid injections within the 6-month preoperative period. Patients who received injections were stratified into biweekly cohorts by the timing of their most recent injection. The 1-year rate of postoperative infection treated by surgical débridement was compared between injection and noninjection cohorts. Univariate logistic regressions of risk factors and a multivariate analysis for patient comorbidities and injection cohorts associated with increased infection risk were conducted.
Results: In the 76,090 TKA patients identified, corticosteroid injection within 2 weeks before TKA increased the risk of postoperative infection (P = 0.02) and injections within 2 to 4 weeks trended toward increased infection in univariate regression. No significant differences were observed in any other injection time frames. In the multivariate analysis, injections within 2 weeks before TKA were identified as an independent risk factor (odds ratio: 2.89; P = 0.04) for postoperative infection. Additional risk factors included chronic obstructive pulmonary disease, coronary artery disease, diabetes, ischemic heart disease, obesity, rheumatoid arthritis, and tobacco, whereas female sex and patient aged older than 65 were protective.
Discussion: Our results suggest that TKA performed within four weeks of a corticosteroid injection may be associated with a higher risk of postoperative infection; however, delaying surgery more than four weeks may not provide additional infection risk reduction. Further prospective randomized studies are needed to determine the optimal timing of TKA after corticosteroid injections.

Hope you have another surgeon lined up. That was pretty of him to neglect to tell you about the 3 month wait. Also the fact your PT was surprised he thought you could fix the tear with physio.
 
Pretty Frustrated right now... Made an appointment with my ortho for tomorrow. He has a nurse call to tell me he can't do surgery for 3 months after my injection... I am fuggin hot. I told him I did not want to do this if it was going to slow me down in anyway getting things fixed. If I am stuck in pain over this for 3 months because he didn't clarify with me I would have to wait 3 months I am going to be irate. Had he told me that I would have gone to get a 2nd opinion elsewhere and found a Dr willing to fix it up front. I told him do not have me spinning my wheels here. This is just so frustrating I could headbutt a Nun!

Now I am looking into PRP which he probably should have offered me first. See if maybe he does that or can give me a referral to someone who will. Also if I have to wait I am definitely going to run BPC-157, and maybe even see about picking up enough GH to run 2IU a day for a while and running a cycle of collagen producing gear. What I am not willing to do is stop working out, or let my entire left side get weak as hell while waiting on this. So annoyed right now...

Oh yeah I did 35 minutes on the treadmill, and my physical therapy exercises during lunch.

No need to excuse the French that is exactly how I feel about it. I even protested a bit when he decided to send me to PT and told him I did not want to be spinning my wheels on this. That he should be damn confident that I will recover with only a shot and some therapy. That I was not confident at all about it.

Agreed, my first surgery they did within about 12 days of my injection. The injection failing to work was the reason we did the MRI and went for the surgery 2 days after getting the results.

Exactly what I had to do last night. Just breathe it out and realize he thought he was giving me the best course of treatment. I do wish he had told me their policy was to wait 3 months after a shot for surgery before doing it though. I would have definitely asked for other options. I am just going to tell him if he sticks to the 3 months I am going to look for another doctor who will take care of it earlier. Also going to print out some studies to take with me like the one I pasted above. Have some ammo so I don't just seem like I am being difficult without any reason.

Going to do a little upper body today after the doctor since I am taking off the rest of the afternoon after I will have some time to go to the gym before it gets too busy.

Interesting I weighed 211.2 today, up a little even though my calories were pretty low yesterday. I did have dairy and carbs before bed so maybe that has me holding more water.

192p, 77f, 260c = 2101 calories
 
You could be holding a bit of water from the cortisol stress of all this news.

Email him those studies and bring copies; a good doctor will be willing to listen at least and weigh the risks vs findings you present.

Good point on the holding water, I didn't think about that but definitely stressing. I am going to head out in the next 45 minutes, I have 2 studies printed out to take with me. One is the one above and another one that basically stated that they found no direct correlation or causation as far as post operative complications due to timing of corticosteroid injections.

I can't speak for the HELIOS yet, as it takes a bit to really do it's thing but the RELAX has really been helping me sleep the last few days. Less pain, and has really just helped me relax and let the stress fall away so I can sleep. I haven't slept this good since I quit smoking the real deal.
 
I was really surprised with Relax and thought I’d have a tough time suggesting it to people as a rep for Apex. I had honestly filed CBD in the snake oil file until giving this a try, but I was happy to find that it was a pretty decent anti inflammatory. At least that’s what I get from it. The nighttime applications are something I look forward to every night. I found that continuous use of both Helios in the am and Relax pm keep my knees feeling good and my stupid shoulder somewhat better. Neither are going to repair a serious injury, but they’re no slouch at calming down inflammation (in my experience).

After I explained to him how often I am scraping against the torn labrum causing extreme and sudden pain he agreed we should not wait and that I was a good candidate since I don't present any of the other risk factors that would make him hold of on it. So now I at least have a date. I already know I will be out of work at least 2 weeks, so I can get my STD set up so I don't run through all of my PTO.

Basically they will go in and clean the labrum up, move the biceps tendon then clean up the RC as well. He is hoping that is all he has to do with the RC, so that my recovery is quicker. However, he will do whatever is needed once he gets in there. So could end up being a lot longer recovery, and another week or so out of the office depending on how the RC tear looks.

That’s great news! Your shoulder experiences are so damn similar to mine. My ortho contemplated relocating the bicep attachment while poking around in my shoulder last year and we now think he should have just done it then. So, it’s good that your doc is doing it right away as it’s really in an odd location naturally. So easy for something in the shoulder to interfere with proper contractions. Glad you have a plan in place now man, and hopefully the combo I sent will keep things somewhat calmed down until you get it fixed up.

Worked everything 4 work sets just focused on contracting the muscles hard and taking all the work sets to the point they were hard work.

I was very happy to have been able to get a decent lift in. Having so many machines to pick from allowed me to just move on if something didn't feel right. I think the mix of the Helios and the CBD are also starting to loosen up the joint a Little by relieving some inflammation.

Yeah, I needed a good session and was so happy my body let me get one in. I can't really focus on consistent progress in numbers or anything with my shoulder injury being so but I can at least make sure I work out to the best of my current ability

Hey this is going the wrong way! LOL Not too worried about it as I know the calories are in the right range. The weight will come down.

Going to do some cardio and my PT exercises here shortly.
 
Didn’t get updates for a few days……man I’m glad you got it worked out with your Ortho and finally have surgery scheduled. It’s good you went in with information on research you had done and you fully understood what you needed. I hate the way some doctors try to give you the runaround and use their protocol as an excuse, and then they act like that’s the way it’s always done. It’s always good to let your Doctor know that you do your own research and hold them accountable.

It was funny, he was actually completely aware of the studies I showed him. He smiled and told me he appreciated me bring those and being an informed patient. Then told me what they were just from the title. He said I wasn't a major risk so they did not have any reason to wait. So his PA basically got me worked up with the 3 month thing for no reason.

Going to do lower body tonight when I get home. Not sure if I will go to the gym or sweat it out in the garage, but hoping for a good one!

Diet has been consistently in the 2100-2200 range this week. Surprised by the scale not moving back to 209.4 yet but again not too worried about it. I know if I stay consistent everything is going to work out as planned.

Curious, how do you guys account for the fats used when roasting veggies?

Ended up taking a walk last night. I started to work lower body but where I tweaked my knee last session was still not ready even after a good warm up. So I decided to take a walk instead and pick up legs today or tack them on tomorrow and do a full body workout.

Shoulder is a little grumpy today as well, must have slept wrong. Woke up stiff and grindy. Glad I have the HELIOS on hand, it loosened up a little from it.
 
Curious, how do you guys account for the fats used when roasting veggies?

The wife got some spray (like Pam) olive oil we use that but very little. (just enough to get some garlic powder to stick to them)
