So even if his arms are 18", for him to be at 19" lean would mean increasing his absolute arm size by an inch AND also getting leaner, which means losing some fat and making up for that lost fat size with even more muscle, so we're probably looking at gaining 1.5-2" or more in actual muscle while losing a lot of fat.
