Blood test tomorrow. What to check?

Hi, I am going for a physical tomorrow. It's been about 4 years. Last time my free test level was just above minimum, but still within range. What should I check the blood for, besides Lipids (cholesterol), liver values, sugar and free test? Insurance is paying for it. I have typical simptoms: fatique, lower sex drive, slow recovery. Thanks in advance for advices.
41 y.o. 11% bf. Been lifting continiously for about 6 years.

bro, i had a blood test done and my liver enzymes were very high. So before you get anxious about it, a lot of us here that lift frequently (lot of muscle damage, etc) will have elevated liver enzymes like AST. mine sure were.

a later ultrasound on my liver showed no problem. obviously i'm not going to stop lifting for 2 weeks just to get a good blood test, there are other ways to test your liver in the case you think there is a problem despite the fact working out has been proven to raise liver enzymes.

My Hepatologist told me bilirubin is the true indicator of how well the liver is functioning. Bilirubin results from the enzymatic breakdown of heme. Unconjugated bilirubin is conjugated with glucuronic acid in hepatocytes to increase its water solubility and is then rapidly transported into bile. The serum conjugated bilirubin level does not become elevated until the liver has lost at least one half of its excretory capacity. Thus, a patient could have obstruction of either the left or right hepatic duct without a rise in the bilirubin level.

Because the secretion of conjugated bilirubin into bile is very rapid in comparison with the conjugation step, healthy persons have almost no detectable conjugated bilirubin in their blood. Liver disease mainly impairs the secretion of conjugated bilirubin into bile. As a result, conjugated bilirubin is rapidly filtered into the urine, where it can be detected by a dipstick test. The finding of bilirubin in urine is a particularly sensitive indicator of the presence of an increased serum conjugated bilirubin level.

When a patient has prolonged, severe biliary obstruction followed by the restoration of bile flow, the serum bilirubin level often declines rapidly for several days and then slowly returns to normal over a period of weeks. The slow phase of bilirubin clearance results from the presence of delta-bilirubin, a form of bilirubin chemically attached to serum albumin. Because albumin has a half-life of three weeks, delta-bilirubin clears much more slowly than bilirubin-glucuronide. Clinical laboratories can measure delta-bilirubin concentrations, but such measurements are usually unnecessary if the physician is aware of the delta-bilirubin phenomenon.

With elevated bilirubin jaundice may occur. What essentially occurs is a buildup of bilirubin, which is formed when red blood cells break down. Since it cannot be excreted in the bile, it essentially backs up into the body and causes a yellowing of skin and eyes. This is not something you want to see!

The normal bilirubin range is 0.1 to 1.2 milligrams per deciliter (mg/dL). The normal range may vary slightly from lab to lab. Normal ranges are usually shown next to your results in the lab report.

Your level of bilirubin may be higher than normal because:

You are taking a medicine that raises the bilirubin level in your blood.
You have an infection.
You have a liver or biliary tract disease, such as hepatitis or gallstones.
You have a blood problem, such as anemia.
A bilirubin level lower than normal is, in general, never cause for concern


The normal range of values for AST (SGOT) is from 5 to 40 units per liter of serum (the liquid part of the blood).

The normal range of values for ALT (SGPT) is from 7 to 56 units per liter of serum


AST (SGOT) and ALT (SGPT) are sensitive indicators of liver damage or injury from different types of disease. But it must be emphasized that higher-than-normal levels of these liver enzymes should not be automatically equated with liver disease. They may mean liver problems or they may not. For example, elevations of these enzymes can occur with muscle damage. The interpretation of elevated AST and ALT levels depends upon the entire clinical evaluation of a patient, and so it is best done by doctors experienced in evaluating liver disease.

It is, therefore, worth mentioning that these liver enzymes do not give an indication of the function of the liver. Sometimes they are mistakenly referred to as “liver function tests” or LFTs, but it is a misnomer commonly used.

Mild to moderate elevations of the liver enzymes are commonplace. They are often unexpectedly encountered on routine blood screening tests in otherwise healthy individuals. The AST and ALT levels in such cases are usually between twice the upper limits of normal and several hundred units/liter

What is usually most helpful is serial testing of AST (SGOT) and ALT (SGPT) over time to determine whether the levels are increasing, remaining stable, or decreasing.
 
Wow, thanks for thorough explanation! Here is what I have:
Bilirubin total...................0.9 g/dl Range 02-1.3
AST...............................34 u/l Range 17-59
ALT................................35 u/l Range 21-72

One thing I noticed that BUN value is at 29 mg/dl, range is 9-20 mg/dl, I attribute it to high protein diet and CRP is at 0.88 mg/l, range is <70, that I think could be because of early arthritis in my shoulder (I have been having those joint pains after bench and shoulder presses. Could be wrong though.
Oh, and I asked doctor to do hormonal panel, the only thing they checked was total testosterone which was at 666 ng/dl with the range of 262-870. When asked why nothing else was checked, I was told that since total T is good then I should not worry about free T, estrogen etc. Hate this crap. Oh, well I guess, I will have to wait untill next physical.

Bilirubin, AST, Alt all fall in the normal range. Elevated bilirubin levels are what you should keep and eye out for.

It's important for you and your doctor to realize that the increases in BUN seen in healthy weightlifters who eat higher protein diets aren't necessarily a function of kidney health but are much more closely correlated with their diet and training.

Elevated CRP is often a sign of arterial inflammation or arthritis. University of California at Berkeley researchers recently showed that taking a daily 500 mg vitamin C supplement can decrease CRP levels by 24 percent. And according to a new study in the Journal of the American College of Nutrition, people low on magnesium are up to two times more likely to have elevated CRP levels than those meeting their quota

The most common testosterone test measures "total" testosterone in your body. The other test measures just the "free" testosterone. This is the testosterone that flows freely in your bloodstream. Physcian's usually will conduct this second test only to confirm low "total" testosterone results.

There are three main types of hepatotoxicity seen clinically from the use of oral steroids and some prohormones. These include intrahepatic cholestasis, peliosis hepatis, and hepatic adenoma. Cholestasis is the most common side effect seen, with peliosis and adenoma being much less common. Cholestasis occurs when bile flow through the liver is impaired. When this happens, products that are normally carried in the bile to the gut and excreted begin to build up within the liver. These products can reach toxic levels and cause cell damage and death. Monitoring for liver enzymes such as AST, ALT, bilirubin could detect if damage is occurring in the liver. Cholesterol metabolism becomes impaired as a result of cholestasis, with an increase in LDL (Bad cholesterol) and decrease in HDL (Good cholesterol). These cholesterol changes may lead to an increased risk of heart disease, and are very common with oral steroid and some prohormones use. HDL below 10ng/dl is not uncommon to hear about. That’s bad considering the desired level is greater than 40ng/dl! Finally, jaundice may occur. Buildup of bilirubin will occur because red blood cells break down and cannot be excreted in the bile. When this happens bile will back up into the body.

Synthesized chemical called a bile acid will help increase bile flow.

Serial testing is the best way to determine whether the levels are increasing, remaining stable, or decreasing.

Chronic elevated liver enzymes should be evaluated by a physician experienced in evaluating liver disease.

Synthesized chemical called a bile acid provides excellent liver support.
 
I read the reviews of Jarrow Formulas Bile Acid Factors and every single customer is happy.

Interesting, I did not know people should take bile acid if they had their gall bladder removed... I still have mine... Now I have to figure out why my bilirubin is high...

The liver produces bile that is stored in the gall bladder. Bile is released by the gall bladder to aid the digestion of fats. It consists of cholesterol dissolved within bile salts. Gallstones occur in the gall bladder as a result of too much cholesterol, or too few bile salts within the bile. The imbalance causes excess cholesterol to separate out of the bile and form stones. Synthesized bile acids (also called bile salts), causes gallstones to dissolve by a mechanism that is not fully understood. It is known to reduce the production of cholesterol by the liver and also to reduce the absorption of cholesterol from the gut. Both of these actions decrease the amount of cholesterol that passes into the bile. Also, since synthesized bile acids is a bile acid itself, it increases the level of bile acids within the bile. The combination of these two factors reverses the imbalance and stops the cholesterol separating out of the bile. The gallstones then begin to dissolve.
 
I've ordered some Jarrow Bile Acid Factors in hopes to improve my bilirubin count. I need to see if it has been this high in the past...

My order of Jarrow Bile Acid Factors arrived today - took one cap with dinner. I really hope this lowers my bilirubin and relieves the weird, dull pain/pressure I sometimes feel under the ribs on my right side. I think I'd be jaundice if it were my liver - I have a feeling it is my gall bladder.
 
Wow, I just got lab results back too and Bilirubin total is over the limit too (1.3), but I have never even gone on a cycle before. My general practitioner said its nothing to worry about. Should I see a liver specialist before I go on my first cycle?

Having elevated liver enzymes is not an indication of a specific liver disease.

While it is not uncommon to have elevated liver enzymes, in order to determine the underlying cause, additional tests are usually necessary.

The first area for jaundice to be noticable is on the frenulum that is below the tongue, but often its missed as its cumbersome and rarely thought of to notice it there, until it appears on sclera thats when people notice it.......

The cause of Liver Pain may be more easily identified by identifying other symptoms, such as fatigue, itching, swollen testes, difficulties breathing, eating problems, or even shoulder pain.

Gall bladder pain has a pattern and usually occurs after meals. The pain may be accompanied by vomiting or sweating. Gall bladder pain can also intensify with physical effort or sudden moves. Pain attacks can last for a few hours and they are usually triggered by the consumption of foods that are rich in fat.
 
I don't have any of the other symptoms. I've been taking the bile acid supplement for three days now and I feel a big difference - the pressure/discomfort/dull pain I was feeling has greatly diminished. I really hope it is not placebo :)
Anyway, if the pain returns I'll be going to the doctor asap. Thanks again for the bile acid info :thumbsup:
 
ok well how about this for a couple liver values if u dont mind looking over it.

Albumin serum 4.3.............range 3.5-4.8
AST/SGOT serum 58..............range 6-40 (???)
Alakline phosphate 55.............range 27-129
ALT/SGPT Serum 170..............range 10-49 (WTF!!! I Havent even started my first cycle yet!!!!)
Total Bilrubin Serum 0.5..........range 0.3-1.2
Cholesterol serum 157...........range 140-200

Now i am bulking right now and working out very hard, I dont drink at all but i do supplement 120 grams of protein a day with a total goal of 300+ a day. This is very worrysome to me seeing how i dont even drink alcohol that my values the AST inpartciular is so high..
