VARICOCELE...can it effect testosterone?

you will need a duplex Doppler ultrasound of the penile arterial and venous systems. This tells u whether the arteries in are okay, which they will be unless u have severe diabetes or atherosclerosis

It will also tell u a bit about the veins. This is more complex. There is a system of valves designed to keep the blood in to initiate and maintain an erection. Damage to these from trauma is a possibility. This may involve more complex x ray tests injecting dye into the veins.

Fracture of the penis is traumatic disruption of the corpora cavernosum, where the blood is held. This is best treated by immediate surgical repair, as delay results in lower success rates.

I suspect u may have some problems in the us with tracking down a urologist with any real experience in this. This type of meclhamical problem and surgery for it is fairly rare, and I doubt within the remit of the average "office " urologist.

This is not my specialist field but I have a mate who is a first class urologist here in the uk. I would be happy to put u in touch with him got further details and advice if u would like.
 
I live in new york city stuartgould. Yes it makes sense these things need surgical repair. And as days go by, lower success rate. Well its already 4.5 years already because i went to urologists 1-2 weeks after the accident hoping it would get better but it did not. So 1 year and 2 years and 4.5 years pretty much means the same thing.

I will see another urologist on next friday and the urologist i saw told me to see this one because he has the equipment to do the duplex ultrasound of the penis. Do you know if they will just do the test while i am there or does that need another schedule?

I really dont care where i go to see a urologist who can fix my problem. This has been making me depressed for the last 4.5 years with nothing done.

Yep it is a simple outpatient procedure that needs no preparation, so they shld b able to do it then and there.

went back to my regular doctor today and he said he is not referring me to any other urologists because those urologists he told me are going to be the office urologists like the one i saw 2 weeks ago.

Next friday i will see the urologist that was recommended by the urologist i just saw b/c he has the equipment to do the penis duplex. Now i can only wait till next friday. Its hard to enjoy life like this since i been like this for 4.5 years.

So I had my varicocele spotted when I was 18 (im almost 22 now) on my left side. Doctor said dont worry about it, until when I was 19 I noticed my nuts started to get rock hard when I got erections, did push ups, walking up the stairs or any other strenuous activity. They had no idea what was going on.

My balls have shrunk noticeably since I was 18. About a few months ago I started to panic and get VERY depressed I had to go to mental clinics and couldnt go to school cuz of it. Urologists (i viisted like 5) all told me I was okay and sperm count came back normal. Im better now but all of a sudden I feel like Im gettin a varicocele forming on my right side now...this scares the hell out of me since my right testes might shrink to left side proportions.


The most important thing Im worried about is of my balls shrinking...

And also, why is everyone here talking about testosterone tests instead of sperm tests? I have a high sex drive, does that mean my testosterone levels are normal? The urologists never talked about testosterone tests to me.
 
I went to see the urologist department today and only got to see the Nurse Practitioner and I told her about my ED problem and the blood testing that I needed done and she quickly started asking me about my background and education etc. I even told her that I had testicular discomfort on the left side she examined my prostate and spermatic cords briefly and said everything was fine and since I didn't have insurance they would only test me for Prolactin and she said it was the only thing that would indicate low libido this is bull**** cause I know otherwise that everything has to be taken into account such as Estradiol and Progesterone on top of all this she was very insincere. If your a young guy and say you have ED they look at you funny and think your making **** up, these docs need to spend some time on online forums and get a good Idea just how many people from 17-40 that have erectile issues. Well all in all this is another drain on my pocket probably for nothing but atleast once the prolactin test is out of the way I will have a better Idea of how to treat myself.

that nurse/doctor whoever you are going to can f*ck off. They need to go back to medical school.

You need to see an endocrinologist thats familiar with andrology. You may want to look around for "anti-aging" clinics or D.O.'s (different from MD)

Have that person order your tests, or call Rhein's and try to order a Rhein's test yourself. Its a full urine panel, recording your piss throughout 24hr period that will tell you EVERYTHING and ANYTHING about your hormones. Its not just a snap shot.

Then return to a urologist...if you sperm count is low consider varicocele surgery. If sperm count is fine, determine from your blood work if you think your Low-T, Low-Libido or whatever your issue, is related to your testicles or your brain....this will be determined from the blood test.


If all else fails, get some money together and go visit Dr. John Crisler in michigan. Trust me, I went to 4 of these f*cking prick doctors with no knowledge of anything but selling viagra for the drug companies. I finally decided I was sick of this crap and pretended I had pain in my testicles so I can get the varicocele surgery and visited Crisler for testosterone recovery.
 
damn my right abdominal area is getting sore from time to time...im tryina feel if im starting to get a right side variocele but im not sure if i have one or not...I mean there is supposed to be a vein u can feel even when u dont have one. but why am i having pain?

i dont think its a hernia..arent u supposed to feel some kinda bulge for hernias? i just have occasional sharp pains close to groin area...
 
another question: if my balls have shrank considerably, will using HCG increase the size of my testicles? I dont think i have a problem with my hormones, but if i take HCG will the extra hormones increase the size?

wow i am crazy. i just read all 34 pages of this topic...

well i think i DO have right sided varicocele forming..which means I will now have BOTH sides...Im plannin on going to a urologist that is located in a hospital where embolization is performed since the other urologists prefer the micro method in their hospitals.

The thing is should i really have this procedure done even though my sperm levels are good, testosterone good (assuming), and have fairly little pain? I do know for sure the left varicocele caused shrinkage of my left and my right testicle. The BIGGEST concern I have is the size of my testes. I do not want my red testicle to shrink more than it has.
i believe its like 4cm

Oh, and should i test my testosterone levels? if so which test should i do? Also, should i lie that I have more pain than I actually do to get the embolization procedure?

First, i wanna thank all the guys involved in thos thread, its the best information ive found in internet regardind this issue.

Im argentinian, 24 years old. About a month ago i found out i have varicocele in the left testicle. That testicle has always been smaller. But i always thought that was normal.

To make things short. Ill give you guys my studies results...

Ultrasound

Testicular size

Right: 29x19x39 mm
Left: 27x20x39 mm

That sound similar in size, but to me the difference is bigger.

I have varicocele grade II

Besides i had an hormonal test too.

Here in Argentina we seem to use another kind of medition. Ill transcript what my tests says.

Estradiol: 20.0 pg/ml
Prolactin: 20.5 ng/ml
FSH: 3.2 mUI/ml
LH: 4.8 mUI/ml
plasmatic testosterone: 5.41 ng/ml
free testosterone: 26.4 picoMol/l
biodisponible testosterone: 6.1 nmol/l

My sperm count was normal but i present a disminution in motility and morphology.


Hormonaly wise the endo said i was in the low normal range. And that a varicocele surgery might improve things. I visited him before knowing all this because i had a very mild case of gyno on my right nipple. It developed during puberty. And from those times i also carried some estrogenic fat pattern. After a cutting that seem to have improved a lot, and now, during bulking, my fat seems to be acummulating on my belly rather than my hips. In my last tests, my estradiol is very low, lower than normal actually. That isnt the case with my prolactin, but its in the normal range. My endo said itd had to be beyond 40 to cause gyno or another complication. My levels could be caused by stress of wich ive had a lot lately. So my conclusion is that during puberty i had some very high levels of estradiol or prolactin. And it caused those problems.

Now, back again to what worries me the most.

The Urologist said hormone levels werent related but i would get a better spem count.

The testosterone thing surprises me. I always had and have a high libido, have had acne, im quite aggresive, motivated, very hairy all over, and for the last years i havent been deppresed at all (except a little now couse of all this ****). I only seem to have to work harder than other guys to get muscle, but thats all. If i compare myself with the majority of men i could say im quite masculine.

The urologist also said that im not getting the left testicle to grow more because atrophy is only avoided during puberty, then its too late.

This particular urologist recommends microsurgery, but i have seen embolization recommended. Wich gives the best results?

Besides that, what do you think about my homonal profile?
 
I finally got my prolactin test results back from the Urologist dept. and it was at 29.9ng/dL way out of range I thing the high end of normal is like 17.5 to 17.7ng/dL and they refered me to see an endo and were suppose to have me an appointment scheduled but havn't heard from them yet. It sucks to have no libido at all looking at a naked woman and not getting excited this is truly hell on earth.
 
I will be going to the urologist who can do a penis duplex tomorrow. Does anyone know if a varicocele can cause erectile dysfunction? I mentioned how hard it is for me to get and maintain an erection and i had to squeeze my legs together to maintain it. someone said it could be because i have low blood circulation in my penis which makes sense.

okay went to this urologist. He did a urine test and a blood test on me which i think will be useless since these tests will be pretty normal for me since i had a urine test with my other urologist before.

when i told him about my penis injury, he did say there is a chance i could have penile fracture and it healed itself. He asked me when i have an erection and before i ejaculate, is my penis still hard or partially hard? I said that first off, i had to squeeze my legs into my testicles in order for me to even get hard and stay hard. I told him then its pretty much partially hard.

He saw my varicocele but we did not talk much about it. He did ask me to come back in 2 weeks where he will conduct more tests. I asked if he was going to do the penis duplex and he said no.

I asked him if he be going to see if there was any structural damage on my penis and he said no. He did say he will inject something into my penis and will give me a machine that will see how my erection is when i am sleeping? Can anyone tell me what machine and procedure he is talking about here?

I still have to wait another 2 weeks and i feel like my life is put on hold forever now.

The machine that they are talking about will take a reading of your erections while you are sleeping its called nocturnal tumescense test, men are suppose to get normal erections during REM sleep. When you go back to see him he will either inject alprostadil or Prostaglandin E1 into your penis to get it to automatically erect then he will probably check to see how your blood flow is. When I went to the Urologist to get my blood work done they would only test me for Prolactin even after I told them I wanted a full hormonal profile done and when the results came back elevated they refered me to an Endo and my earliest appointment is August 14, so I definately know they are giving me the run around. They told me that they would set the appointment up for me and call me back after they got it arranged well I had to call them back cause they never returned my call and now that they have made their money they don't give a **** so they turn it over to another doctor to help him make some money. I have come to the conclusion that they want to milk you and the insurance companies as much as they can before they benefit you in anyway possible. In your case he could have given you that shot today in your penis and ran that test with you walking away with some answers but he wanted to make a fortune by prolonging the visits.

Yes it is suppose to suppose to be legitmate if your penis get fully erect after that injection then that means your trouble is some other reason than a vascular deformity. If scar tissue is the reason from keeping you from erection then they have creams that will soften it up over time by being applied periodically it would be the same ones that they use to treat pyronies with for those guys that have a crooked penis. Before I had anytype of surgery from this guy I would check his background a little bit before I proceeded with it cause some guys have been messed up for life after having revascularization done to their penis. I think you have alot of room for improvement simply by softening the scar tissue that you may have. If the varicocele didn't shrink your nuts I wouldn't worry about having that operated on unless it causes you pain and discomfort.
 
Since your nuts did shrink because of the varicocele then the surgery may be a good option. It may be a good idea to take care of the varicocele first but since you doctor did blood work you will atleast know where you stand with your hormones. He can put you on HCG to help get your nuts back to size and of course if that fails you will probably have to go on Androgel permanently. Do you still have a sex drive even though you can't use your penis? This would be an indicater that you still have good T levels.
 
The urologist i first saw sent me to a place to do ultrasound of my testicles. When i had that done, i got the results and thus it shows i have a varicocele. What do you mean i had blood work done? The urologist i saw yesterday just did a urine test and blood test from my right arm but no results yet. Those test are useless b/c it should be normal for me.

I have never had sex in my life msl2009 because of my erection problem.

Can you tell me what my next steps should be after i see him in 13 days? You seem like you know a ton about this.
 
Great discussion on this forum! I'm 27, with a grade 3 varicocele that I didn't give a second thought about until the spring of '08 when I played an unplanned round of golf in boxers. Left testicle hurt for about 3 days so I made a trip to the ER and then urologist. They said if the pain went away it was nothing to worry about. Well the pain DID go away, maybe it wasn't even related to the varicocele. But the idea of having this large varicocele has seriously been wearing me down mentally for over a year.

I've spent hours and hours researching, and Embolization almost seems too good to be true. Still, I'm very uneasy about the chance of coil migration.

Has anyone gotten the scoop on migration from a radiologist face-to-face? If you have an active lifestyle (lifting, running, exerting yourself), is this more of a concern? I'd really like to be rid of this varicocele without open surgery.
 
^ I have the same concern as you.

i have an appointment with a radiologist on thursday (i didnt even bother going to a urologist since he probably would reccomend surgery and not embolization)...

I asked the radiologist over the phone about the migration and he seemed positive it would never migrate and that hes done over a 100 of them and has 35+ years of coil experience...

i have one on both sides btw

When you go back to see the doctor in 13 days just be honest and tell him that its your money and that you are paying him for a service to be done. Tell him either you can do it or you can't. Ask him straight up am I waisting my time or not. Most of all tell him to go ahead and inject your penis with the prostaglandin or alprostadil which ever he uses to get your penis erected then he can check with the equipment that he has for blood flow while your penis is erect. You will know for sure after that visit if you have problems with the veins or not. The blood test is still nescessary to see where your testosterone levels are since your testicles had shrinkage from the varicocele.

I meant injecting your penis and getting it to erect and checking to see how your blood flow is. Thats something he should have done when you went to see him the first time, if he had atleast done that then you would have been ready for the duplex on the very next visit. The whole reason to see a doc is not only to get healed but leave his office with answers not being just as clueless as you were when you went in. I'm just angry with these type of people cause the way they have done others and the quality of treatment that I am getting. I'm already out 242 dollars for one visit with nothing acomplished and I can't even see the next doc that I'm suppose to see until 3 1/2 weeks.

The thing is i never been with a woman before but if i cannot get my penis hard when i am alone in a room where i have no anxiety, how can i get erect with a woman? This penis injury i had back in 2005 pretty much messed up my life.
 
does shrinking of ur testicles mean you have to have low testosterone? Because my testicles have shrunk considerably but my testosterone i think is normal. and this is from the varicocele.

I'm in tennessee currently out of work and have no insurance so I may have to halt further visits for now. Its sound like you do have erectile dysfunction from the way you have described in earlier posts. I only wish I could go back 4 years and know what I did now I would have never engaged in the so called harmless act of masturbation again that has left me almost impotent with no libido.

Your testosterone level could possibly still show up in the normal range with shrunken testicles. I'm sure it would be considerably lower though. Do you still have a sex drive? Can you still get a full erection? Do you wake up with morning wood? Can you get spontaneous erections? These are all indications that your testosterone is still in the good. There are other hormones that play a crucial role as well cause there are some guys with high testosterone but low sex drive (libido).

The blood specimen that your doctor took on that last visit should be for your testosterone level. He should have your results for you on that next visit.

yeah i get erections, morning wood and i have good sex drive. however my varicocele is giving me pain and I get random body pains all over my body, especially near my groin area like abdominum area, and even in my rear end. the pain is like little sharp pains, i know in fact its a blood flow problem.

My testicles actually get rock hard when I do certain things like walking up the stairs, pushups, and get an erection (i dont mean the sack)...i had this happen to me about 3 months after i found out i had a left side varicocele, so im guessing the varicocele might be causing this hardness?

I am going to get an embolization done on both sides. I choose this method because i dont want to get my body opened up again and they would have to make two cuts to fix both sides while the embolization can do it through one. plus i dont want them to accidentally mess up with my artery or somethin...

I overmasurbated for years but it only got really excessive the past couple of years and I had a bout of adrenal fatigue from when I was working back during the winter my libido was really low then but I had been suffering weaker erections since about late 2007 but libido was fine then. Once I got laid off back in february I had cold hands and feet constantly with sweaty palms,insomnia and anxiety signs of adrenal exhaustion. About a week off from work one day I masturbated for about 4 hours straight without ejaculating while looking at porn and then my penis went completely limp. I tried to get it back erect but it wouldn't so then I finished off and ejaculated on a flacid penis. After this it drew up short and small for about a whole week also had burning sensations down there as well then it finally went back to its normal size flaccid. Once I got over the adrenal exhaustion and my penis went back to normal my sex drive is no longer there. I can look at porn but it doesn't turn me on. I have had sex a couple times since then but my erections are really weak and I have to hand stimulate to get it up and as for the sex it didn't really do much for me or her, very dissapointing. I look forward to the day if I ever see it again that I wake up with morning wood and that I can look at a woman and get a full erections spontaneously. I pretty much avoid women now since I have been like this hardly ever do I go out in public much. I get depressed when I go to places like the supermarket and I get looks from women knowing that if I hadn't masturbated compulsively and disrupted my hormones that I would have been in a relationship. I do believe that if I had taken the opportunities that I had in the past to be in a full time relationship then I wouldn't be where I'm at today having to live life in the shadows.
 
Sorry to hear about your pain Jordan2345..... as long as you got a sex drive and you can get good solid erections then I wouldn't worry to much about my testosterone levels. You can take Borage oil to help with the inflamation, its not a miracle cure but it will help a little till you get the embolization procedure done.
 
msl how old are you now? Man this stuff has messed up my life so much. I never got to go out and do things because of this accident. Penis still hurts when i touch it and i have E.D. Varicocele grade 3 which i will need to have some procedure done but i dont know why urologists dont give a damn when i told them my testicles have shrunk. Do you know if there is a chance that i have E.D because of the varicocele which caused my testicles to shrink? I read online and urologist told me no way.

Yes it could be possible to have ED because of the Varicocele. I've read several other forums and lots of guys out there are making the same claim whether medical doctors will admit it or not. I think the reason most Urologist are scared to do this procedure is becuase they are afraid of lawsuits if something goes faulty with the surgery and some of them are just wanting to collect your money telling you everything is fine hoping never to see you again. I will be 32 next month. I have had major ED for the last 6-7 months now although I had weaker erections for 2 years now but before I had a sex drive a libido but now I'm not getting erections simply by looking at naked women almost complete anhedonia. I could make it through the rest of my life in a content manner if I had my libido back and atleast 30 to 50% of my erectile capacity that I lost restored.

for the most part, I know my testicles shrunk and I know my testosterone dramatically decreased
 
Update

Hi all,

A quick update from me. Note I'm in the uk.

A recap, I had the emoblisation surgery which was unsucessful, then the keyhole surgery to tie the vein off in my groin about 5 months ago. This was the end of the variococele, I'm pleased with the results and the vein is still there but shrinking.

As for the other symptoms that seem to point to low T or Thyroid issues I've since had countless blood and urine tests and been referred numerous times. My thyoid tests are fine and I was told my T levels are quite raised so no issue there. I recently saw another urologist who said that it's time to be referred to an Endocronologist so hoping that this will be the end of it.

I recently tried Clomid again out of interest and this made no difference again even though for about three weeks before my surgery it made me feel normal again. I'm going to try Formex out of interest while I'm waiting for the Endo appointment to come through but 18 months of this is now getting very frustrating.

I'd be interested Stuart in whether in your professional opinion Varicoceles can cause the symptoms that so many of us are reporting or is this just a massive coincidence? My urologists were convinced that it just wasn't possible.

You should still get copies of your bloodwork never just rely on what the doctor tells you. Just because they said it came back normal you could be within low normal range and still have symptoms.
 
AS I told before i got subinguinal surgery ( testicle brought to surface and than veins ligated ). the testicle grew 5 mm in length so it grew a little. the surgery was 2.5 month ago. Now I think my veins start to show larger again as there is definetely something is going on in the scrotum again. I felt some pain from time to time in veins after surgery so maybe they enlarged again. I dont do any hard lifting now so odnt know what is going on. that was my second surgery. the firs was 7 years ago.
now my sperm is very bad in motility and count -and my total test is way low -below down limit - 6.5 nmol/L . but my SHBG is below down limit, but my free test is ok ( in the midle of range ) in % it is 3 %. that was the same results before this surgery.
My LH is around 6 IU/L and FSH is around 7.5 IU/L so lh and fsh are good in the normal range, but I was thinking maybe surgery will brought my total test back -nothing.
I went to color doppler again and it was shown a lot smaller vein dilation on valsalva maneuver.- and there told me that surgery was done well, but I am not sure if all of dilated veins was ligated because I can still see many of them !
my left testicle is going back to smaller so I am going to visit uro again to see if other veins are showing and what is going on.
so my testicle respond and gre a little but now it start to shrinking again.....
but all in all I feel good and no muscular lost or any simptoms- dont have erections every morning but maybe it is normal. I have hair all over me no gyno, some fat on abdominal area but not to much.
I want to brought back speramtogenesis as oone can figure out where is the problem besides normal LH and FSH.
 
also is it possibly to sue a urologist..man i went to this russian lady and i told her i had a varciocele problem and my nuts were shrinking and getting hard...and she told me to get these two blood tests that are NOT testosterone test...i believe they are like karykotype or something..and it came out to be over a THOUSAND dollars! for a damn blood test? plus she got to my appointment like 20 minutes late and when i asked her a question at the end she just walked out the room...worst doctor ever

my insurance covered it but it still came out to be 800...WTF
 
Guys should i fix my penis first my e.d and all those problems or the varicocele first? The urologist i am seeing now is only concerned about the penis. He will be injecting something into my penis next friday. All i am doing now is waiting and life is just at a stop for me. I am worried about my varicocele grade 3 since my testicles have shrunk.
