Sudden testicle shrinkage. No libido, fatigue. Varicocele?

No lower back pain or burning, no redness around genitals.
Two weeks after it happened, my left crotch began to feel uncomfortable, like something was pulling on something else. After some months, it developed into a pain, kind of hot sharp feeling in my left groin. I guess that is the varicocele. However, they also found I have a small inguinal hernia which they say might cause the pain.
I have it almost constantly, have become used it it.

Inguinal hernia can cause erectile disfunction as well...but i think you know this already..i have a friend how had ED from hernia and after the surgery he felt like 18 again...:)i don t think it matters if it s small or big ,i strongly believe that if you correct this 2 problems you will be fine again

Another thing, before it all happened, for several weeks I had very high prolactin, way beyond what has been measured in my blood results.
It was high enough to cause acute sensitivity in my left nipple and chest, but without any noticeable change in the tissue.
Then my balls shrank and two weeks after that, my chest became half the sensitivity. Two months after was when they first took my blood for a test. Ludicrous that they didn't do it immediately really, it could have told a lot.
Even with the sensitive chest though, my libido was fine at that time, until the overnight libido drop to nothing and testicular shrinkage to about 40-50% of volume.

Had an mri in January of this year, no sign of pituitary abnormalities.

2 thing you need to do go to an endo and scan your thyroid and to an uro to scan your kidneys..the kidneys because you urinate blood and if there is any infections that might be the reason for high prolactin as well

Take some zinc and macuna pruriens to lower your prolactin levels naturally and see how you feel

Endo ran thyroid tests in march, were a little high - did a manual check and found no physical signs. Had an ecography of my kidneys last June or so after the urine blood happened, nothing found on the scans.
Infection causing high prolactin is interesting, but wouldn't it show up on blood tests? High white blood cell count or something.

If it s an infection on your urinary tract it wont show up in blood test...only in urine test..the first thing doctor should do in your case ...did he tooked an urine sample from you ?

Thryve sampled a few urine tests, no sign of any problems apart from the blood that one day - they think that might have been from dehydration or a muscular tear or something. Serious things were eliminated with the kidney and bladder ecographies.

The only option right now is to fix your varicocele and hydrocele..do you feel like you have a bag of worms or are your testicles hang too low? Even if your varicocele and hydrocele are small they can still do a lot of damage to your testicles/hormones...because your blood flow is restricted to that area...even if the doctor say no...you are the only one who knows your body better ..and my opinion is to try ..you have nothing to lose

My testicles and scrotum are almost always near my body now - when they were healthy, they hung freely. Since 2 weeks after it happened, I've been able to trace a couple of veins on the surface of the left testicle. The cord area from which it hangs is thicker too. It is a grade 2 varicocele apparently.

Thats what they said to me before i found a smart one and said that i need to fix it because it messed up with my hormones and my sperm also...thing confirm by my endo too..so i m waiting my surgery in january and hope everything comes back to normal

They have more or less ignored everything I have told them or has been revealed in tests. "Variocele does nothing, causes no pain or has any effect at all", "small hernia, nothing to worry about", "blood results are only a bit high", "all within normal ranges", "testicles seem fine", "it's all in your head".

The last urologist referred me off to a psychologist, who then lost my appointment.

I think varicoceles can clearly cause problems with testicles, due to the used blood not flowing out properly. Can also have further effects on adrenals as the veins are connected, which
might explain my flatlined emotions.
Hernia I have read can also cause problems, compressing blood vessels.
The only part they seemed to pay attention to was the high prolactin, and the TSH they mildly noticed.
I need to find a good urologist with experience.

Varicocele is known to cause hypothyroidism, TRH is released from the testicles and hypothalamus so this would make sense that you have elevated tsh
Varicocele stressed the andrenals and lowed TRH production and this results in elevated TSH ,it s one theory i ve seen on another site
TRH will also increase and stimulate prolactin in hypothyroidism that will contribute to sexual dysfunction

Any idea why I would have had very high prolactin before the sudden testicular shrinkage and libido nullification?
My theory is a combination of physical and mental stressors.
I think that with the testicle problem caused the hormone imbalance to be irreparable for my body. Like it was derailed.

Are you sure you want the laparoscopy varicocelectomy?
I read quite a bit about it last year, inguinal and subinguinal microsurgical with an amplification of 400, I think, were the most successful.

I m from Romania and here are only a few doctors who perform microsurgical surgery.
Embolisation is the other option but every doctor a saw said the laparoscopic surgery is much more succesful ..embolisation is better for recovery period that s all.
As i said your tsh is very high...optimal range is 1.5-1.8to a maximum accepted 3.5,you need to retest it along with t3 and t4
Usually high tsh comes along with high proclactin
Do some research about high tsh and high prolactin

Where could I find optimal levels for the various hormones.
Doctors seem so far unconcerned with optimal, when it seems to be quite important.
Briefly looking, high TSH and prolactin can be related to hypothyroidism and prolactinoma. MRI was negative.

Good luck with your surgery.

Fweed my man, my story is exactly the same as yours. After the summer vacation a 20 yeard old me, was feeling fantastically good. Never ever I felt so good and helathy. And than one day I wake up feeling as if I have the worst flu in the world, felt like dying SRS. Than went to take a shower and saw my penis all shriveled into my body and hard as stone. 2 weeks later i develop a huge varicocele. And now 1,5 years past I my left testicle shrank a lot. In addition my neck became shorter shoulders got narrower, while my hips got much much wider. I lost 1 inch of height too. My feet size is smaller too.

I HAVE NO IDEA, What is happening to me. Legit went from a superhuman to a smierable old man.

How is your situation now bro?

That sounds really similar, apart from my testicle shrank very quickly, half volume after a few days.
My penis is almost always like it has been put into a cold bath, as in no extra blood stays in it while flaccid.

Your last line is how I feel. I was strong, fit, constantly horny, erections from a few seconds of thinking about a girl, 20+ minutes of morning wood without stimulation, never tired, defined muscles and vascular arms in particular.

Immediately after it happened I was exhausted, falling asleep in the early afternoon like a 90 year old man.
No libido, no morning erections, no sexual fantasies, no sexual dreams, and after some months noticed my muscles shrinking and six months later my veins were all smooth and hidden for the first time in my adult life.

Maybe the same thing happened to both of us.
Which tests have you had done, have you had the varicocele repaired?
What have urologists said?

A possibility for both of us could be that the vein going up from the testes into the abdomen became compressed and caused a varicocele that our bodies just couldn't handle.
It is called "the nutcracker effect". But doctors don't agree on the effects of varicoceles.

My erections have become more difficult to obtain as the last 2 years have passed, no doctor or specialist has given a clear answer, no diagnosis or treatment.
I am going to book an appointment in a capital city where there are more experienced specialists, but it is still luck whether I find anyone.

We can compare info and maybe find a common factor.

I m currently fighting with my prostatitis with diffrent antibiotics with small improvements ..i ve also read that varicocele cause some prostate problems ..anyway i found one interesting article and you should read it entirily

It is a positive sign that you have seen some improvement.

Your link didn't post, as it was on your reading list, but I will search for that title.

For me nothing has changed, so i will try some specialists in other places.
I think I should also discuss having surgery to repair the varicocele and hernia.

I wish you a lot of luck with your varicocele repair, and that it leads you back to full health. Tell me how it goes. Some people say they notice improvements after a couple of weeks.

Fweed, we are not alone. It took me awhile to find people on the internet that have similar stories to ours. However, recently I found a forum where guys have somewhat the same situation as ours. They call it Hard Flaccid, like their penises are dead and lack proper blood flow. And a lot of them have varicocele and testicle shrinkages. This guys discussed their problems since year 2010. However, as I was reading through the thread, I found that ca. in year 2012 they found out what was wrong with them. Basically, we most probably, have a severe case of CPPS. It is a muscle spasm of our pelvic floor. A big/significant part of the blood supply and nerves, going down into the groin, goes through the pelvic floor. However, when the pelvic floor gets cramped it presses on this crucial arteries veins and nerves and basically strangles the groin area.

It's also been found that just as many cases of so-called "varicoceles" have no vascular anomaly at all, and are caused by simple muscular congestion and inflammation. This is why they almost always occur on the left side, the blood vessels on that side travel a far messier path than on the right. This is also why almost all patients who undergo surgery for them end up going under the knife again for a new one shortly after. The valves in a normal young adult's veins don't just randomly fail, and only in one very specific part of the body. It's absurd. THE VEINS DOWN THERE, DONT EVEN HAVE VALVES! Think about it for a sec.

It's also basic scientific fact that hindering the blood flow to and from your testicles (via CPPS, muscular tension, etc) is going to have an impact on fertility and hormone levels, yet 99% of doctors will sooner condemn a man to a lifetime of hormone replacement drugs instead of fixing the actual problem with physical therapy.

My question might sound too personal for you, but man it is important to know, do you kegel a lot when having sexual stimulation? Now when I think about it whenever I have sexual stimulation I would kegel, and my kegel muscles would be too strong. In addition, since this started, right after, I couldn't kegel, I couldn't stop my urine. Like the muscles down there were dead. But now I understand, they weren't dead. Its just they were so cramped and locked, that there was no way to flex them. Like imagine curling your biceps, and then trying to curl more, there is an anatomical limit, as to how much it can curl. And now imagine this biceps in this super contracted and pulled state for a long long time.

Considering the subject, it isn't such a personal question.
I had been doing them for a few years, never straining - I thought. More as an exercise, once or twice a day.
How can this tension last two years, or be permanent.
My pelvic muscles became much weaker, almost ineffective. I can stop urine but not like I used to. It barely moves my penis when I do it.
Read about another guy before who said the same, before it would make it move up and down, afterwards it just kind of went into his body a bit when he tried. Same with mine.

I found reference to hard flaccid before, but the site was gone.
I'll look again.

My varicocele stuff became noticeable only two weeks AFTER when my balls shrank and I had lost my libido.
I thought that was weird. The veins became prominent on my left testicle. At first I thought it was vitality returning to them,
rather than the opposite.

By the sounds of it, you really do have plevic floor congestion or imbalance. Not all muscles might be cramped, some of them might become short and weak. Please if You are able, I advice to got to a Physical Therapist. Let him/her check you, see if you have any probelsm down there with your muscles. You are not being able to control the muscles donw there its a big sign. And after you developing a varicocele its a big sign too. See what you can do at home, is hindi squats. Look it up in the internet. Do this whenever you can. YOU WILL GET BETTER Just from this exrecise alone. Im going my 3. day and I feel much much better.

I had left testicle problems. pain, shrinkage and retraction. Had to stop TRT. Had urinary tract infection then went to urologist and it was epididmyis. Turns out there is 32 ft. of cord behind each testicle. 1 cord is swelled up. I have this problem from trauma I had over 35 yrs. ago.

It s been 3 days since my bilateral laparoscopic varicocelectomy,i have pain in my abdomen and my right testicle but i m amazed how the majority of the veins are gone...my testicles look more bigger ...anyway it s too soon to jump to any conclusions ..doc sais to wait at least 3 months to see radical changes .

Also on the size of the testicle, if it is temporary swelling or if it regrows.

I didnt know i had 2 varicoceles,my doc said that he saw during the surgery that i had a begining of a varicocele on the right too
For now...it s hurts like hell(the surgery area)..my testes are not swollen ..only a discomfort on the right ..they really look bigger ..but as i said it s soon to tell,no libido for now(the pain kills it):))
Hope to recover soon and see the results

Did he take care of both of the varicoceles? Maybe the testes have increased inside because of the blood not having so many veins to escape from, so the pressure has gone up slightly.
Would be good if it is permanent and adds to function.

Hy bro,yes eventually they are swollen,my right is more than the left,yes he did fix them both,now i can walk and do my things with less pain then 2 days ago,the only problem i have is that my penis is smaller ..lifeless...my doc sais it is normal because of the surgical stress and it will be fine ...and the testes will heal in 2 3 weeks ...
Regarding the libido..i only have 2 morning erections since my surgery but my last one was really weak...maybe i need some time to heal completely

Yes, give it some time. Soft tissue takes a few weeks to heal like he said. Keep watching and relax, eat good food and let your body repair.

2 weeks after surgery:sex is better,erections very good...it s easy now to keep it hard..my penis when flaccid looks bigger it s like there is more blood flow
The problems with my prostate it s almoust gone
Better urine flow
Sperm is more dense
I m not sensitive to cold anymore and chills are gone
Hope it will stay this way!

A little bit yes but not much..maybe they need time i don t really know...as far as my actual condition i m really happy...i hope it will stay this way ...i will do some blood test and a sperm test 4 months from now to see if there is some changes .
My T was low below 500
My sperm count was low
Anyway i dont regret i get this fixed it seems it can cause a lot of trouble from low t ,prostate hypertrophy to andrenal dysfunction etc..
2 weeks from surgery and today was my first training day...i did a light training but it felt good:)

That's great Razvant, a lot of change in only 2 weeks.
Keep writing any changes and further improvements you notice, it is really useful to know how treatment goes - that information is hard to find on the internet, successes.
When is your next blood test?

If it works this well, it should have been the first thing they did for me 2 years ago.

How is the pain?

Believe me i wish i did this surgery back in the summer but my first urologist said it s nothing to worry about,my second one told me to test my sperm my hormones and then he sent me to a great sergeon to get it fixed.
I will test my hormones and sperm 4 months from now as my uro said...i have to wait from 3 to 6 month to see some real improvements.
First week i was si scared because my penis looks like it was dead..verry small when flaccid ,lifeless no sex drive no nothing ..but then when i had sex with my gf i was like..wow..a really strong erection ..
And another thing i discover is that before the surgery i was cold all the time ..when i drink coffe and take pre workout stimulants my penis and testicles shrink all the time..but now they dont ..they look fuller ..no more chills...maybe there was something wrong with my adrenals too because of my varicoceles

My penis has been smaller and colder to the touch while flaccid since this happened. For me, the dead penis, no sex drive etc is how it has been for me for 2 years.

From what I have read, it is either the DHT or nitric oxide that affects the flaccid blood flow. Could also affect your body temperature or at least how you feel temperature, due to the capillary blood flow.
Maybe also thyroid can affect it - since the endocrine system is all linked, I am not sure about what causes what.
But if it has changed after your operation, it could be the bloodflow to your adrenals returned to nearer normal, as the vein from the testicles joins with the vein going to the adrenals. Another guy had been talking about that connection.

It is good your life is going back towards normal.

Hey man, have you had the sperm test? No real updates from me apart from things seem to be worsening in terms of functionality. I don't know what my next course of action should be.

I'm 36, and before this was totally healthy with a high libido and excellent erection quality. How can that break in a day with no clear cause?
I feel like a crippled 90 year old and after 2 years of this I am losing hope.
In the beginning I was scared, but doctors told me it wasn't serious, not to worry. Yet they did nothing, found no cause and solved nothing. Several even saying it was in my head, about 50% of them in fact, urologists and Endocrinologists, simply because THEY couldn't be bothered to figure out what had happened. Why did my balls shrink, no one can answer that it seems.

Hey bro,no not yet..aftet 6 moths from surgery...my sperm looks better ,i feel more energic ..but my libido almoust the same ..a bit better but not how it use to be...
After the surgery i had really strong erections but after 1 month they fall down again..don t know what to think anymore..i will retest my t and others in march
I m glad i did the surgery because my testicles looks better my sperm is dense but no major improvements on erections ..despite i feel mire energic than before the surgery

Good that you feel generally better in terms of energy.
Have you thought about pelvic floor muscle tightness? I read it can be a factor. Do you have any pain or discomfort at all.
One guy I read about followed his varicocele surgery with a self treatment of two drugs - HCG and another one, one for 3 weeks, one for 5 weeks, to kick start the testicle production.
He was also weight lifting, sleeping very well, taking food supplements. It was working for him.
However, I have heard conflicting things about HCG, so I don't know about it.

Try other things too, it may be that the variocele surgery is the first positive step.

I have to do an MRI for pelvic muscle to see..i tried hcg before surgery but it raised my estrogen too much..my endo want to try again after surgery i will see her next month,what is the other one?

I have thought that I need a pelvic mri scan too, to see what is messed up.
I don't remember the other medicine, I am sure I saved the website link somewhere, but I haven't found it again so far. It was a year or so ago - the guy posted a daily update on a forum for months detailing his progress from varicocele surgery to being normal again.
He was very active and methodical about his cure.
He stopped the drugs after a few weeks and continued with supplements, sleep, exercise and his state continued to improve. Had high libido, energy and things were working.

I dont think i have pelvic problems but my uro said that i should do it anyway..i think our varicocele affected our hormones for too long ..i will try again with hcg to see maybe its better this time
