Baseline Bloods Puzzling me

Wide

New member
Hi guys

I recently found this website. After some reading through for a while I would like to pose my blood work to you for your valued opinions.

I am a 30yo male in Australia. Feel in good health. Train regularly.
Weigh 103kg, Height 186cm waist 93cm. (gained 38kg in 10 years mostly muscle )
BF ? never tested. Was and have never been "fat" hence never tested
NO cycle history

I decided to do some baseline bloods a few years ago.
I noted in subsequent bloods my T levels we falling.
This years ones concerned me as the T levels were approaching single digits.

I apologize in advance for the detailed bloods, but from reading the posts here I have realized detailed bloods are the way to go.
I have ommited cholesterol fasting BSLs and general liver and electrolytes as everything is in the "normal" range.

I saw an Endo earlier in the year whom "reassurred" me and said everything is "fine" after briefly examining me ( ie taking my BP and weighing me ).

I have sought another doctor here whom seems more interested.

Clinically, I am tired more than usual last couple years despite reduced training volume, improving diet and much more relaxed work hours and stress levels.
Libido is satisfactory.

concerns of note
1) T levels falling,
2) SHBG levels on the lower end.
3) DHEAs on lower end for my age
4) 17 OH pregnenolone twice upper limit of lab reference
5) highish fasting cortisol levels

As I understand it the pregnenolone is used to make cortisol and DHEA eventually.

Any suggestions as to the likely process, and possible solutions are much appreciated.

I have attached the bloods in a doc as typing them here would look messy. Am happy to type here if thats the thing to do. :)

I notice Prolactin is highish. That will reduce T. Ejaculation within a day of giving blood can result in higher Prolactin test results. i.e. don't do that before your blood test. Also FWIW about 50% of high prolactin test results are not confirmed with repeated testing.

LH is low. Are you on a diet/have you been dieting? Hypocaloric diets reduce LH and this T.

Thanks for the fast reply mark.

Interesting about the prolactin. The docs weren't concerned about it or comment on it at all. There was no funny business the night before any of the tests.

No diet before any of the bloods either man. Docs again said the LH and FSH levels were satisfactory, although LH 1 seems like there isn't much signaling going on. I understand LH secretion is pulsatile, but could I be getting it at the trough everytime its tested I wonder.

Never been on a calorie restricted diet either man.

Thank you JanSZ
Thats an impressive list indeed. Love your work

I'll do the expanded thyroid profile next week + order a kit locally for the Cortisol and DHEA saliva testing AM/PM.

The links you provided are very helpful. Thanks again.
