Hello! 44 yrs old male been working out for 24 yrs. Not sure if im posting in the correct forum.

Testosterone -290
Free test- 10.9
Glucose - 112
Triglycerides- 207
Cholesterol - 255
Ldl - 167

Numbers are high. I am actually decent shape 26% body fat. Arms , chest cut but huge stomach. Always bloated looking. Im 180 lbs

There is a trt dr at the gym i work out at recommended 3 months test cypionate, 3 months glp , 3 month growth hormone releasing peptide.

Welcome to the forum. I’m new here too. My total testosterone was a little lower than yours at 219 May 2025 which caused similar issues with fat gain around the midsection like you are experiencing. Today total T is at 1234 on TRT and feel great. Bodyfat started at an estimated 33% and now down to 25%. Muscle size, libido, sleep and strength all improved considerably.

I also started Tesamorelin 1.5 months ago. Positive changes have been slowly appearing weight is down to 217 from 225 as of today due to Tesa, TRT, diet, and exercise.

I’m not a fan of GLP-1 due to the appetite suppressant effect but it has worked for many if food is an issue for you.

Your gym doctor sounds more like a trt clinics legal drug dealer, and a idiot...... TRT is a life commitment, not a 3 month protocol, and unless your fairly short, at 180lbs you probably do not drastically over eat.

It's likely that your low total and free test is accompanied by high estrogen. You probably/very likely are also dealing with insulin resistance.

Cleaning up your diet and dropping bodyfat will help improve all of those things, test, estrogen and insulin sensitivity.

Your blood work doesn't mention your LH, FSH, Thyroid or glucose. That information would be helpful. It would tell us if your capable of making your own hormones still or if TRT is the best option, and the thyroid and glucose might tell us more about the big stomach problem. Low thyroid (hypothyroidism) slows metabolism, lowers your other hormones and causes fluid retention in the gut/stomach area too
 
Hey man. Welcome to the site. Based off those labs your triglycerides, cholesterol, and ( I assume ) fasting glucose suggest possible metabolic syndrome/insulin resistance. Have you had an A1C done recently? I'd be hesitant to start trt, a growth hormone secretagogue, and a glp-1 all at the same time. It would be difficult to figure out what was doing what and if side effects occur, what compound was causing it. Is this your first hormonal workup and were these taken in the morning preferably fasted and before 10 am? Somewhat lean arms and shoulders with a large abdomen can also indicate insulin resistance. Going down from 26% bf to a healthier bf may improve those blood markers back into normal range and bring testosterone up without TRT. Like mentioned, TRT is for life, so not to be taken lightly, but definitely an option if bf drop doesnt improve numbers. I'd stay away from any GH secret/GH initially especially with your current glucose and triglyceride numbers.
 
Great information. I was thinking same thing with trt dr. Something not right. All the research trt lifelong. I think he is trying to get me on it
