Kidney transplant

I do. I have kidney disease. But a very healthy transplant right now. I was just hoping someone else would be on here so we could compare notes like creatinine, protein intake, salt intake and overall lifestyle with the meds we take (prednisone, tacrolimus... etc)
And all those in regards to maximizing training and making sure to keep kidney strong.

Any here with kidney disease?

When was the last time you had your kidney function checked? Especially given the lifestyle most of us lifters live.

Most people dont even know they have kidney disease until you're in stage 3 kidney failure.


Any kidney experts on here? Any knowledge on what could harm our kidneys regarding these supplements we take? Something more than just dont take NSAIDs and dont get dehydrated.


Would love some knowledge or some fellow lifters with hurt kidneys.

I have. Very recently actually, December 2019. On PD for 14 months before that.

In terms of supplements, you have to be VERY careful since you are taking Tacrolimus. As you have already been instructed, no grapefruit, seville oranges, star fruit, Earl Gray Tea, etc. all of these can effect levels of the enzyme CYP3A4 which metabolizes Tacrolimus (and other medications). Many flavonoids, flavolnols, etc. found in supplements can act as inhibitors or agonists of CYP3A4 and other Cytochrome P450 superfamily of enzymes. Since none of these (even the best standardized formulas) have remotely the precision of dosing compared to actual pharmaceuticals, it would make balancing your Tac levels impossible. That risks either rejection (to low) or damage to your new kidney (to high). In my opinion there are too many unknowns and risks with many of the herbal based products. Even if they are standardized for an ingredient not know to effect CYP3A4 (high probability some products aren’t even researched for that) they could still contain trace amounts of flavonoids that do. This is not a slam or fear mongering on these products. Many are well researched and safe for the vast majority of people (several I recommend for athletes I coach) but for guys like us, best to use an abundance of caution.

Happy to keep up the thread or move to message. I’m not as active right now on the forum both due the the transplant/bloods/clinic back at work, and dealing with a lot of disappointed wrestlers who saw their season cut short due to COVID19.
 
Thanks for the comment. Dandelion is a natural diuretic how could that be good for my kidney? Do you have any literature I can read on this?
Just from Google and reading reviews on Amazon ppl who have kidney disease.. I have organic dandelion leaf and root tea.. seems to be helping me.. I'm 3rd stage..
 
I'm on trt I quit my aromasin also I wont take arimidex.. I read some ppl develop kidney disease from these drugs..I'm getting tested in three weeks to see if I have made improvements.. keto diet works well.. and no more than 700mg of salt per meal..no red meat.. no taurine whatsoever or creatine.. gallon of water daily for me

I would agree to avoid this personally. Since you have a new kidney, you have likely already shed any water weight you may have had. Additionally, diuretics could alter electrolytes one way or another (whether or not they spare potassium) Stick with your Rx’d water intake. As you know, you need to try and monitor your urine output as a reduction could indicate a problem. Taking a diuretic would skew what’s happening.

Yes..I agree...i still have mine..idk how a person with a transplant would respond
 
I'm 1 year post transplant. Am on trt 150 test cyp 1x a week. Red blood cells were a problem but they are under control now. (After 8 phlebotomys). Blood pressure is good (not on any bp meds)and creatinine is 1.2-1.4 which is normal.

Anti rejection meds-
5mg prednisone
Cellcept
4mg tacrolimus

Supplements I take that were approved by my transplant team-
Orgain plant based protein
Beet root extract
Vitamin d

How about you guys?
 
Keeping blood pressure low seems to be the priority, after maintaining adequate hydration.

Beet, astragalus, grape seed, garlic.

No transplant here, but Dante Trudel said high dosed Astragalus seems to help guys filtration on bloodwork. I take 3g or so daily preventatively.

White willow Bark
Black cherry extract
Tumeric

Careful with the White Willow Bark. It’s a natural NSAID. Unfortunately, Tumeric is a no go if you are on Tacrolimus.

I should be asking you for advise! Not quite 4 months for me. I had some complications (lymphocele) that resulted in first a JP Drain installed and finally another surgery with a double fenestration into the peritoneum to drain the lymphatic fluid. So to some degree I’m still recovering physically. BP issues resolved with transplant and average 110/74 (my previous elevated BP was due to ESRD). My creatinine is actually right in your range but has been as low as 1.0 (4 days after transplant) and as high as 1.5. My increased creatinine seems to correlate with the lymphocele (now resolved) and elevated Tacrolimus. The later of which they are still adjusting.

Meds:
5mg Prednisone
1500mg Mycophenolate mofetil (Cellcept)
3.5mg Tacrolimus (Prograf)
400/80mg Sulfamethoxazole/trimethoprim (Bactrim)
450mg Valganciclovir (Valcyte)

Will come off Valcyte end of the month and Bactrim after a year. Due to the elevated creatinine (and other side effects like mental fog and badly, badly shedding hair) they would like to move me off Tacrolimus and onto once monthly IV Belatacept (Nulojix) however, the COVID19 crisis may put this plan on hold.

For now, my sup routine is limited to Magnesium and Vitamin D. Still researching what is not verboten.

Interesting to hear about taking you off tacro and on to an IV once a month. Keep us updated on that. You sound like you're on the same drug regimen I'm on. Can I ask where you got your transplant?
