Interpret results of a raging alcoholic

Hi, 36 yr old male here, slightly overweight with history of alcohol abuse which caused significant liver damage. However, I'm 8 months sober so I decided to check where I am. Also i am taking antidepressants like sertima/Wellbutrin./voxitine and ritalin. I still don't know if it is because of ageing or SSRI's lowering my erection quality and libido.

Total Testosterone 646.08ng/dL (164.94-753)
Free Testosterone(CLIA)- 16.60pg/mL (15.99-50.00)
SHBG(CLIA)- 54nmol/L (14.55-94.64)
Estradiol 37.72pg/mL(<39.80)
Albumin (BCG)- 4.60g/dL (3.20-4.80)

Should I ask for TRT therapy to my doctor?

TRT with a 646 total?? No Dr. in his/her right mind would prescribe it.
If you have one that is out of his/her mind you might be on to something.

646 is an excellent natural number... If I were you my goal would be to somehow get off the anti-depressants somehow.
(If possible, I know its hard).
(Microdosing, etc)...

100% I know jack **** about blood work but seeing as everything seems to be in range (althoug the range these days is pretty fucked) I would think the pills is the first step. Sauna, Cold Plunge, red light therapy, social groups, get out in the great outdoors, really focus on your mental health... I know it is hard, I suffer with very bad depression, battle daily with suicidal thoughts BUT I also have a strong voice inside me who tells me to push on, the above help me tremendously.

Can also look in jelqing and penis pump for libido and erection quality, definitely works well.

How much were you drinking? Any other drugs along w it? When I was drinking a half pint of vodka and snorting 1-2 grams of coke on work days and a liter of vodka and 2-3 grams on non work days, my body’s natural serotonin was absolutely dismantled. It was a steep hill to climb to feel “normal” whatever that is. I second the notion that the anti-depressants are playing a role w drive and quality of wood. Talk to your Dr about scaling back dosages but you need to do some type of physical activity to create an endorphin release. Start w walking outdoors, be in the sun for vitamin D, do something that makes you feel better about you. You got this!!!

I have been seeing a number of guys at exactly your age refer to it as aging lately and I always think that's a bit weird. I'm 41 and still feel 16. Some of the guys I look up to around here on the forums are in their 50s and 60s and still setting PRs. Ligaments and recovery periods aside, I believe age is mostly in our head. I will say my current stack has libido through the roof though:

anabolic effect, optimize t, orchic extract.
And any time I feel off, I throw in Alpha Gel. The sleep on Alpha Gel is amazing and the morning wood is beyond obnoxious.

Can also look in jelqing and penis pump for libido and erection quality, definitely works well.
I don't know anything about the science involved but realistically I have zero doubt that anything boosting bloodflow like that would help.

48 years here and I feel like I'm 17.

Alcoholic / addict here sober for almost 10 years. Great blood work! The first one I would try to cut back on is the amphetamine (Ritalin). Alcoholics anonymous saved my life and showed me how to get rid of my depression, anxiety and live life happily without drugs or alcohol.

It helped my erections go from like 6/10 to easily 9/10 without any supplements to help blood flow. That in itself increased my libido, whether it was placebo or not, It worked.... and there are thousands/hundreds of thousands of people that have boasted similar benefits. You just have to do it right and not over do it.

The pills are probably a culprit with libido. In a perfect world you lower/drop whatever from that possible, if possible.

That being said, dropping a little bodyfat & taking a supplement to knock your SHBG down is absolutely within your power either way. Boron or a product with stinging nettle extract are things to look into - lowering SHBG will raise your free testosterone.

So maybe you cut out the amphetamine (stimulants restrict bloodflow), work on diet & sleep, and get your free test up. Estrogen will also come down some if bodyfat goes down.

I was a bing drinker.Like sometimes I remain sober for a week or two, then I break my sobriety my continouslv drinking 24 hrs/ day for 10 -15 days or even a month. I was jaundiced 4 times. Last scan showed major scarring on liver. After getting sober, I went into depression and took the anti-depressant route. Doctor already warned me libido issues on SSRI"s. I also take klonopin and nitrapezam/ambien. for anxiety and sleep.

What are these two products? I’ve been sober two years and take low dose SSRIs but things aren’t good lately. Been on TRT for years but went from doc script to just handling it myself. In the process of trying to get back to a doc.

It would be worth checking your prolactin next time you get labs. Personally I would ask for a PDE-5 Inhibitor, viagra, cialis(can be daily) or something similar. These DO NOT have to be for life. Use them while you continue to work on getting healthier. Time will likely improve your situation if you do all the things necessary: diet, training.

I do not know how many AD meds you have tried, nor do I know your doses. Discuss leaning more heavily on the DNRI class(welbutrin) and possible lowering doses or swapping for different SSRI's that MAY have less negative effects on your erection quality.
