Golfer's Elbow, Tennis Elbow or Cubital Tunnel Syndrome?

Hey guys, couldn't find an injury section, so excuse me if I post this in the wrong place.

Also sorry for the wall of text, just trying to be as detailed as possible, and I would really appreciate if someone could help, as this problem frustrates me a ton.

I've read for hours upon hours, and I still can't understand what my problem is.

It all started 2 years ago, when I woke up with an acute pain in my right elbow. I never felt anything bad during my training that could indicate a serious injury but when I woke up the next morning the pain was pretty intense. I went to a physiotherapist and he told me to back off from weight training completely and MAINLY from wrist extension exercises (I assume he diagnosed me with tennis elbow) for a month, and also prescribed me some anti-imflammatory pills/gels. Followed his instructions for 1- 2months, and my elbow got progressively better.

After 4-5 months I started going to the gym again. Fastforward to today, the pain is still not completely gone, I train mainly with high reps and can do most exercises, with no pain.

My problem is that I can't pipoint what my problem is, and how to fix it. At some point I thought I had lateral epicondilitys (tennis elbow) because every gripping exercise hurted my lateral epicondyle, I can't do hammer curls because they kill my elbow, unless I go really high reps 15+.

BUT, if I try to flex my right arm really hard, I feel a tingling, slightly uncomfortable pain in my MEDIAL epicondyle which confuses me a ton. (golfers elbow maybe? Cubital Tunnel Syndrome maybe?)

And as if these weren't enough, I've noticed that my right arm ulnar nerve feels pretty weird too. For example if I try to do the Doorway Subscapularis Stretch , my right arm goes numb/tingles. (again, Cubital Tunnel Syndrome?)

So I really don't know what my problem is, and where to focus in order to fix it. If you asked me what caused my injury, I have 2 possible answers, since I never felt a single pain during training.
1) 1.5 month prior to injuring my elbow, I started doing wrist extension exercises to failure almost daily, because I wanted to beef up my forearms, silly me.
2) Skullcrushers always bothered my elbows, but never backed off, again silly me.

So one of these 2 scenarios or a mix of both cause my elbow pain.

What would you say is my problem, and how should I approach it in order to fix it? Thanks a ton!

The first thing I might say is, to try and do all the trigger points (painful spots thru the chain) in the muscles etc. of your upper (bi and tri) and lower (all the forearm muscles top and underside) arms. Then some stretching.
Some good magnesium supp may help to relax the tension too.

When my elbows went south, I fixed them with the following:

1. Stopped pullups and dips for a few months.
2. Spent hours and hours tinkering with pressing width. In the end, I must have adjusted my ohp grip about a while inch for each hand and it made all the difference in the world.
3. 100-rep sets of curls once to twice a week - I preferred hammer, but some prefer barbell. I just used 10lb dumbbells, seriously.
4. Massaging the muscles around the elbow, particularly triceps. Rolling them out on a barbell is awesome.
5. Zero triceps work outside of presses.

A couple months of that turned it around and as long as I get a set or two of 100-rep curls in a month, I don't get any pain at all.
