First time blood work - a question!

I'm going in to get my blood work done for the first time in a couple of weeks so I have my numbers for the new year. Can I get some suggestions of what specifically to ask for just to ensure that I have everything I might need?

I'm thinking:

Testosterone
Free testosterone (and % free)
Progesterone
Estradiol
LH
FSH
Prolactin

Anything I might be missing? Thanks!

I would also get:

SHBG (sex hormone binding globulin)
T3
T4
TSH

and perhaps inhibin (hormone sent from follicular cells in testes to hypothalamus saying that FSH is doing its job and sperm is indeed being produced).

Also, I would reccomend a blood lipid panel (triglycerides, LDL, HDL, total cholesterol).

I go thru my insurance kaiser. Just tell them you want a complete blood panel, and all of those listed should be covered, at least they were with me. Except shbg.
