One week past 19 years old. Total testosterone only at 437ng/dl. Zero strength gains in over 15 months. Zero libido. Zero morning erections. Cannot gain appreciable muscle despite a guaranteed 6550 calories daily.

Is 437ng/dl very low for barely 19?

You need to know more than TT.
FT, SHBG, DHEAs, E2, LH, TSH, to start with.
FT3, FT4, TT3, TT4, Prolactin, DHT, Cortisol (preferably saliva), D3(active and storage), would also be nice to know.

If you have secondary hypogonadism (low LH) there are many possible reasons: I'll list a few.
OPIATES, excessive alcohol, starvation(obviously not your case), AAS/PH use or previous use,
Head injury, phytoestrogens (soy, ginseng), pituitary tumor, excessive exercise, STRESS, sleep deprivation,
high E2 levels.

Some good reference points and that is pretty low for guy your agehttp://www.artofmanliness.com/2013/01/16/normal-testosterone-levels/

Testosteorne as mentioned by itself is useless, you also need to factor in IGF, thyroid, adrenals, and nutritional deficiency as well as SHBG
Since working on peoples IGF-1 levels I have seen a 50-75% increase in guys testosterone. SLEEP is the biggest regulator of IGF-1 levels in males.
So first thing you need to do address lifestyles, Your 19 years old and typical american this age is going to college and partying their azz off, drinking like a fish and doing god knows what. I would not go any further until those are evaluated. To many people are trying to blame it on their hormones, when you are your worst enemy..

My igf-1 was normal. GH was 350% higher than normal. Serum cortisol was very low at 1:35pm at 6(ref range 5-25); but my 24-hour urine analysis for cortisol came back pretty high 24 hour cortisol was something like 79(ref range 0-50). I'm going in Tuesday morning Dexamethasone suppression test, as I've been on inhaled corticosteroids my entire life, and was on up to 80mg prednisone ED when I was younger(puberty age).

I was also deficient in vitamin D. If I fix my cortisol and Vitamin D should my testosterone swing back into a good range most likely? I rarely ever drink or party.

You need to fix the underlying cause then it will go back, not just these 2 variables.
If you are high or cortisol which means you may be low as your may be pissing it out faster then you can use it. High on urine may mean inability to keep up the demand resulting in lower levels. This is why using saliva cortisol from a respected labs only 3 of I suggest are clinically accurate.

You may be breaking it down faster then making which results in potential low cortisol levels.
Why I suggested saliva 4 point. Only labs (neuroscience, lab corp, ZRT,) I use other are inaccurate. I have seen a few thousands to have to come to this conclusion.

So even though my cortisol 24 hours is high my actual serum free cortisol is low most of the time your saying?

Test and find out...
