Blood work issue

I am having symptoms of high E. My stomach feels bloated all the time. I workout all the time. Eat well. I simply cannot lose weight around stomach and its about 10-15 pounds. Shortness of breath after a set of bench press for example. Only happened past year . Moderate increase in Blood pressure. I have used Pro hormones years ago without a decent pCT. I developed gyno about 4 years ago and now it flares quite a bit. Not sure why. HEre is what I do know. I have talked to someone about TRT and they cannot treat me because of high levels

Total t + free = 869
Free T = 10.1 %
SBHG = 69.9 (High)
Glucose was a little high and ALT was just a little high

Estradoil 27.7
Cortisol AM 13.7(probably high)

I was told I don't have much free T and somehow SHBG is high

Would these side effects I have be caused by this? Please let me know

I also took propecia when I was 22 until about 31 or so. I quit because I thought it was causing side effects. I am 37 now.

Any help would be appreciated. Any advice on a doctor or someone that could help me solve the issues.

Ur shbg is crazy high, which is why ur free test is low (shbg binds up test and renders it useless, which is why the free test is a more important number). Estradiol is in range. You’d have to post the cortisol ref ranges for me to comment on that. Cortisol can cause fat gain/bloat, so that’s worth fixing if it’s an issue. Prolactin is also something worth getting checked; it could be contributing to ur gyno flares. Tsh, t3, rt3, t4 (thyroid values) are all worth looking into as well.

yes, so what to do about this issue? Most docs would just say I am good. My symptoms don't say so.
Cortisol AM 13.7 Range from labextension/LabCorp is 6.2 - 19.4
Thyroid
TSH - 1.310 range .450-4.5
T4 8.9 range 4.5 - 12.0
Triiodothyronine, Free, Serum 4.2 range 2.0 - 4.4
T4, free 1.57 range .82 - 1.77

Again, I know nothing about levels and they all seem in this range for the blood test, but I know with this other blood work and this, things could be out of range with thyroid? Do you think a regular doctor would see something since most are in range? Any advice on helping this issue and getting treatment to help me feel better.
 
A testosterone guy that I found online said they don't treat people with total test that high. However, my free is so low compared and SHBG is high. Main issues are bloating, fluid rentention, weight gain, not a whole lot of energy. Gut health is bad as well. I feel like I eat ok, and nothing helps me at all. My heart rate seems great and never had any issues getting weight off. Since I have had these symptoms, blood pressure has been slightly elevated. I also had my stomach looked at, and they cannot find anything wrong. Side effects seem like estrogen or thyroid/cortisol symptoms

A few things that were high
GLucose 104 range 65-99
Bun 21 range 6 - 20
Alt 49 Range 0-44

I don't drink hardly at all. I workout. Don't smoke. Pretty healthy person all my life and always in shape

Your free test and shbg need fixed, those are your main issues. Get those fixed, and I think you’ll notice a difference. Any real doctor wont prescribe trt unless ur 300ng/dl or lower. Your total test is very very good; lower that shbg, your free test will be way better. Free test is what’s useful. Total test doesn’t mean anything if your shbg is high. If your doctor couldn’t help lower ur shbg, find a new one.
 
I’d also get prolactin checked. Ur estradiol is in range, so I don’t see that being an issue. Gyno can be permanent without surgery. Some people will have success getting rid of it with specific drugs such as letro, ralox, caber, etc.

Prolactin would cause puffy nips and lactation. Real gyno is from estrogen; real gyno will involve the development of breast tissue. It will also effect other hormones negatively. Idk a whole lot about thyroid, but I personally didn’t see anything that stood out to me.

I can’t answer that. It will depend on the knowledge of the physician. Endos are generally a better route, but that doesn’t mean that you wont get a stubborn/dumb endo either lol.

I’d get the shbg and free test fixed and see how you feel. That seems to be your main issue, especially when reflecting on your bloodwork.
 
What is your hematocrit level? High hematocrit indicates “thick” blood and can result in being out of breath and increased BP.
 
No. Not overweight. I can’t lose the 10-12 pounds I gained since test results and high shbg. Do you not think that could an issue. I mean it states it’s high and I stated my symptoms. Never said I was overweight
 
Hematocrit looks great. Is it that your cardio aerobic capacity is low? Shortness of breath would be explained if you capacity is low.
 
No. Just really small when I get done with a set of bench press. Nothing major. If I run a mile, it doesn’t seem as bad
