Homebrewing Test C - 1 of 4 vials crashing?

I homebrewed 4 vials of Test C 300mg/ml, 1.2% BA 15% BB, MCT Oil. 1 of 4 vials, all in the same bag in the same drawer, is fully crashing, like completely solid. I boiled it for 5 min yesterday and it immediately recreashed 24 hours later. I'm guessing that more of the raws ended up in this vial and that's why it's crashing? I heated it pretty high, 209F, and I swished it around like 20 different times as it was melting, then i stirred it with a syringe and needle a whole bunch before filtering. Looked super clear no oil texture or bubbles or anything. Wondering if anyone has experienced this and has some tips.
 
Now the vial I've been using for a week just fully crashed over night. Weird. It is like 2 degrees colder today. Maybe I'm pushing too close to the max concentration but the 250mg/ml vials i brewed before crystalized a little as well in 2/4 vials. Really worried I'm not getting an even distribution between all 4 vials.
 
Your BA looks low to me. No idea if thatd make a difference to the crashing but Im used to seeing 2-3% BA (not saying 2-3% is better or not, again just what Im used to seeing). I thinlk your BA/BB ratio is out (the BB should be higher too).

Probably my BA and BB are a little low. After reading more about cyp it seems like it has a lower max concentration than E. 300 might have been too high. At least a 3rd vial is now crashing so I'm less worried about it not being mixed evenly. I'm going to add another 1% BA and 5-10% BB after reheating a vial. Luckily i have sterile vials of each so I dont need to filter.
