Can you have itching OUTSIDE of the nipple like an inch away localized to that area that may be signs of gyno or estrogen sensitivity or is it only sensitivity and itchiness of and behind the nip itself? I know this one is kinda stupid but I’m trying to settle an argument.

It's possible.

I feel like this would be more of a sign of skin irritation than anything.

Remember.. Feelings can travel along nerves. Ever have an itch, and you scratch it, only to find out that wasn't really the spot that needed scratching at all? LOL it happens.
How likely? Who knows? It would obviously be more likely that an itch is where it feels like it is.

Possible yes, probably, not really. Your boobies don’t really work like that. You’ll feel it where you feel it because something is either growing, getting receptor activations etc etc. when my gyno flares up I feel it around the nipple, but I also feel it pretty far from the nipple, in small lumps that hurt to touch because the receptors are getting estrogen and the lumps in that area are growing.
