How to fix corticosteroid induced insulin resistance? Just out drug it?

I have found out that the source of all my problems is that long term low dose corticosteroid use for my lungs has given me system wide insulin resistance at the muscular level, and minorly at the adipose level. My doctor has started to try and wean me off the steriods and put me on Clomid for the time being. I made some nice strength gains--but haven't gained any real size despite eating 7,000+ calories a day.

Has anyone experienced anything like this? I sort of feel like my best bet is to find a doctor who will monitor my health as I out-drug the underlying problem. I think 400 or 450mg Test E per week is the most that is "legally" allowed in the U.S for "hormone replacement therapy". So I suppose I should try and find a good sports performance/ medicine doctor that understands the situation and can right me the script and watch my vitals as I try and see if more test will fix my symptoms(allow me to make gains--despite poor insulin usage in my body).

Any thoughts guys?
