Sounds like serum sickness. It's an over reaction from your immune system to the androgen and estrogen increase. Sometimes the high solvents in UG gear does it too. I'm very sensitive to serum sickness even after 20 yrs and have to step up and step down dosages. Guess the pyramid thing in...
I've been on TRT over 15 yrs and it's been a blessing. It won't cure all the anxiety but it certainly helps keep mine at bay.
*For a point of reference my natural levels were 100ng/mL before TRT. Getting my serum level to 400s made a big difference but I prefer being around 800s for quality...
Nolva works but Clomid has a better track record for jump starting the HPTA. Don't go overboard on dosing with any of these things since you never know how you'll react. I'm still a lower dose guy 20 yrs later.
What are you LH/FSH levels? Have you tried restarting your HPTA? As for cycle dosing I'd start with 250-300mg weekly. People underestimate the power of accurately dosed test.
I have similar results and requirements dosing once per week. I moved to 20mg daily injections and maintain serum levels above 600ng/dL daily. I piss my TRT out really quickly and constantly have to chase a new balance.
The averages the lab list could easily be completely made up numbers too. You're a little low but that is probably due to age and previous use of AAS. What were your LH/FSH? If they're high it means your testes aren't responding well to the stimulus.
We need the lab ranges too because they can differ between companies even when using similar units of measurement. Your free T looks a little low and SHBG is on the high normal side.
You need blood work to figure this out until you learn enough about your body to go by feel. I've been on TRT a long time and still get it wrong sometimes without blood work.,
Gels tend to promote more DHT conversion than injections so that might be what your experiences in the differences. I don't absorb my TRT for crap sub-q and do shallow IM with insulin needles.
For me this was a pregnenolone issue. I started supplementing 400mg the for the first week and it made a huge difference. hCG should help this issue and DHEA for me is more of an energy boost. The DHEA we had then 300mg per serving hence the dose.
