Lab results 27 year old male, any input appeciated

Hi everyone,

First post on AM but been reading on here for quite awhile. I recently went to my doc to get some bloods done and included testosterone and thyroid as part of my lab work. I’ve been experiencing fatigue, low energy/lethargy and reduced sex drive (pretty careless mentality about it) than what I’m used to for over a year now. I don’t smoke, rarely drink, and haven’t done a single cycle. Been training naturally since 2013. I sleep great too and the main reason I went to my doc was for the lack of energy.

Attached are my results. I thought total test seemed low for someone my age. These were taken at 8:30am. Any input would be appreciated.

Total is a little low, but Free isn't too bad, not great either. Add to that your TSH is high, and with you feeling sluggish, it sounds like your metabolism is a bit hampered from that as well. Unfortunately @ 399 you are not likely to find a doctor to put you on TRT unless you go through one of the websites where you pay cash monthly.

What did your doctor say about the bloods?

Thanks for the reply. He said overall they looked good. Testosterone in his opinion was good. We’re rechecking thyroid again soon. I had it checked a year ago and it was within range so it may be a temporary thing.

Overall, everything came back good. I just wanted to communities opinion on results for my age. Maybe I just need more sleep and dial in the diet a tad more in regards to lethargy.

It could be job related too. I work in the IT field. Minimum 8 hrs a day and am mentally drained a lot of the time.

If the androgen receptors are very sensitive sure, but otherwise 399 is pretty low for a young man. He's only 27 complaining of lack of energy and libido with the test level of a 45 year old, so I would be wanting to know why.

TSH is normally high when on gear but for a complete natty that is a sign of something out of balance.

Yeah if you optimize rest and diet as well as nutrition you can probably bring a lot of that into range, or improve where it is in the range. I am glad you are going back in to check on the FSH since you said you are sluggish. Not sure what you do in the IT field but you got to learn to let that stuff roll off of you. I have been in IT for all of my professional career and you can either let it bother you or just learn not to internalize any of the issues you have to deal with, and leave it at work when you leave any time you are not on call.

Will try to optimize by getting more rest and watching diet a little more. Overall though I do pretty good.

Also, would a protocol like 12.5mg of clomid e3d for a month or so help any of these symptoms in anyone’s opinion?

What are your FSH, LH and E2 levels?

These were not included when the doctor ordered labs. I asked about E2 after I got my labs back and he said “he would not expect it to be elevated” and did not offer any further testing.

I sort of just got the typical “everything looks good” response.

Your TSH is high and that is a symptom of other issues. Your thyroid is sluggish at minimum and probably needs further evaluation. A thyroid not working properly will cause low energy and low testosterone issues.

Doctor mentioned fatigue and sexual function could be affected by thyroid but didn’t mention testosterone levels. Anyways he said we will re-evaluate in 3-4 months. In August 2016 my reading was 1.34 which was right within range.

Testosterone is low normal, but lacking the proper estradiol test and other supporting labs

Symptoms could be manifestation of a deeper root cause only ruled out by proper detail history, other clinical data

Giving testosterone is this situation is pre-mature and not appropriate as it may be masking deeper issues.

What ever happen to differential diagnosis by exclusion ...sheesh

I’m going to assume, being in IT, you stare at computer screens most of the day?

Sounds like his Doctor does not value quality of life and deems no need for further diagnosis. Offering up I will check again in 4 months, now go be miserable until then... Talk about a crappy doctor that needs fired.

OP, you are the boss, your Doctor works for you... insist that he prioritize figuring this out or let him know you need him to release your medical records when you find a doctor who values your quality of life appropriately. It really is as simple as that.

The matrix - agreed. I’m a little perturbed he’s not willing to look into anything else or consider doing other additional tests to provide more of a transparent baseline of my numbers across the board while I’m feeling like this. It seems like a symptom of laziness or lack of caring on his end.

Thor70 - yes sir. I’m on two big computer screens 8 hrs a day sometimes more.

Mrkleen - agreed. I’m going to ask to be recommended to a specialist and take it from there. I’m not an expert but I think there can be some more somone could do to narrow down further why I’m feeling like this.

Not sure where you live, but find an integrative health Dr. They research and treat the cause, not the symptoms like regular dr’s.

Yeah probably right. I just haven’t had the time to research another doc right now. Maybe this weekend I can do some research.
Yeah, the doctor works for you and if dismissive about things that are important to you then they don't have your best interest and are using you as a rotational source of income only....

I’m going to ask him about the following tests just to see how he reacts. I don’t think asking those to be ordered is unreasonable at all in my opinion do you? I have really good insurance so I don’t care about cost. If I was a doctor I’d want a full baseline of everything.

Even if I come back and rested high in TSH I’d almost want a second opinion before I started taking anything.

Tell him you want them run, do not ask. You are the boss. Would you ask your waiter if it was okay to order a burger? No, you would just order it and expect the service to be provided. If he says no then tell him okay. Thank him for his service, up until then and inform him it will be your last visit. That you will be sending for your medical records once you locate your new doctor. You will have orders for the tests in your hands when you leave.
