Chronic rhomboid problems / Bad posture

So I've always been a bit of a sloucher or slumper throughout most of my adult life. Nothing drastic, bur it is clear that I don't have the greatest posture. Every now and then I get rhomboid spasms that take days to go away, and I think it's from overuse of my shoulders. I've recently began doing more exercises to help strengthen the rhomboids specifically, but how do I strengthen them when I keep getting muscle spasms and knots from working them? I'm hoping that this will just go away after awhile through proper training, but has anyone else had this kind of issue with their back, and more specifically their rhomboids from bad posture? I foam roll often, have a lacrosse ball for the knots, but it just keeps happening again and again. Thanks for any help.
