It completely depends on the quality of the extract. The only hawthorn extract which has even been proven to have the BP lowering and heart healthy effects is extract that is standardized to a minimum of 1.8% vitexin, otherwise you'd be using a placebo. Get a standardized extract and dose 2x per day at 600-900mg. With a majority of people, 600mg works fine - as long as it's standardized extract.
