Here's little planner I reverse engineered from one I saw on a different site (I was bored at work). I never use the thing but I thought someone might get some use out of it. It's got cool charts and graphs. The other one I saw wasn't very customizable. The macro percentages for bulking cutting and maintenance were locked. This one is totally customizable, just enter your weight, body fat, and desired macro percentages. I.E. 40/40/20, 50/20/30, Etc...
