OK - Will try to make this as short as possible. Please bare with me and provide as much feedback as possible. Thank you.

I am 24 years old. Had to stop playing football in March of '03 due to bulging discs in my back. Wanted to build some muscle to help rehab the problem so I visited a nutrition store. The guy gave me M1T and Androdiol Select 300 and said to take them both to put on some serious muscle...I was a naive 19 year old so I didn't realize what I had gotten into until a year later when I looked up the stuff on these forums!!!

I took them for about 4 weeks and then stopped for a few months...then took the M1T again for about 2 weeks..and haven't used any prohormones since.

GYNO - Around 6 months later my nipple hurt and I formed a small bump underneath it. It hasn't gotten any bigger in the past couple years, but it is still sore from time to time, and I can squeeze a whitish discharge out of it if I want to. Gives me a fatty, puffy nipple look which I really hate.

LOW TEST AND LIBIDO - I finally got some bloodwork done recently because I have had a feeling I had low test levels these past couple years. It was nearly impossible to maintain constant erections like I should be able to. And I wasn't seeing the gains in the gym like I should for how much work I was putting in. Also feel like I have the body of a 40 year old (no offense) due to constant aches and pains in the knees, shoulders, and back (that still isn't fixed).

Varicocele - I have had several "worms" near my left testicle for a few years...not sure exactly when it formed. I am pretty sure my testes have shrunk slightly since the M1t and 300. They seem to be the same size, like large grapes, but I get dull aches almost every day in that area. Did some reading on here, and saw that some say it has an effect on TEST levels and other fertility factors.

Initial test (1/23/2008):

This test was done at 6:30 p.m. so I am expecting my new test to show higher TEST levels, but not sure how much higher. Thyroid Panel, CBC, Metabolic Panel, and Lipid Profile were all within normal ranges.

Obviously WAY low for a 24 yo so I went to an ENDO and had more tests done.

When you get all your tests, post them here in tabular form, post:

There is not much you can do to increase natural T production.
With your current LH and FSH you may want to try HCG, but leave this for latter.
I suspect that you have high E2 but wait for results.
If you have high E2, then you may want to try Arimidex to get your E2 to the desirable level.
That may increase your TotalT since less of T will be used to produce E2.

All of my testing is done through Quest, and I requested the Estradiol, male, ultrasensitive assay like you said to. I will post the rest of my results as soon as I get them.

What did you mean when you said "There is not much you can do to increase natural T production."

Well I just saw another Endo...he was older and seemed very knowledgeable, but he couldn't decide if he knew what was wrong with me. He said he doesn't think the prohormones I took would have these lasting effects (what I was thinking but wasn't sure).

He was leaning towards Kallmann's Syndrome but I can smell just fine and I didn't have delayed puberty at all...so he decided to refer me to another Endo who is supposed to be an international expert on what I am dealing with.

I just made the appointment with the new Doc...March 13th.

Pretty certain I have some type of Secondary Hypogonadism. Anybody out there who has gone through this and had similar test results or symptoms??? How did you treat it???

ANY MORE HELPFUL ADVICE WOULD BE GREATLY APPRECIATED!!!

Sounds like steroid induced secondary. Talk to your doctor about using clomid or another SERM. This will help restart your natural testosterone. I am in the middle of this right now, even though I am 10 years older than you and never used steroids. Don't let them talk you into TRT without first trying to restart your natural test.

I also recently had a varicocele embolization... nearly painless procedure and it's almost gone, would do it again in a heartbeat. Still waiting to see if it helps my hormones. Good luck!

Testosterone, DHEA, FSH, LH, Prolactin, and Cortisol are all way too low for my age.

Estrogen is in their range of "130 or less" but I just checked and the normal range for men is 10 - 50! So that is way too high just like I expected. (explains tender nips)

So...what does everyone think??? Does it look like I can restart my natural Test production just by taking a SERM for a while...or is it more complicated than that???

With your high estradiol level, you might do well with a strong AI (aromatase inhibitor). As you bring your estradiol down, your body should compensate by making testosterone. I don't think OTC AI's (like 6-OXO, formestane) would be strong enough for you. Like JanSz mentioned, talk to your doc about a prescription AI like arimidex. You are young, I think you can restart... definately worth a try before TRT.

One more thing... low prolactin and cortisol aren't necessarily bad news, as long as they're still in range. DHEA is easy to supplement... sublingual 25mg/day brought mine up to high-normal.

At the moment I would say that you are LOW on estrodial.
So DO NOT START aromatase inhibitors.

Get this test at Quest, exactly as written:

When you get resuts,
your goal is
FreeE2(0.45 - 0.6), best being close to the top (0.6) or (0.6 - 0.7)

All in all, I do not know what to tell you.
I will tell you any how.
Normal course of action for you would be an effort to restart HPTA axis.
If you want, spend some time on it.
If you get fixed, thats good.
If not, you have two choices.
Choice #1, use 380iu HCG EOD (for life)
Choice #2, get from Genova Diagnostics test NutrEval
may fish out some other problems. Highly recomended.

I missed that, didn't notice it was total estrogens vs. estradiol (E2). So quest is the only reliable test?

Well, not everybody is able to get that particular E2 test.
The minimum is that it have to be E2 test, his was TotalEstrogens.
The other minimum is that it have to be "ultrasensitive".
I use the one that I posted.
Problem is that doctors are always hurrying while doing anything.

I would love to run all those tests. The problem is, if you're too young for medicare, most insurance plans will not cover it. The uninsured cost would probably be like $5000!! :blink:

I am feeling better, but I don't know if the varicocele repair has helped my hormones yet. It has definately eliminated some pain, and sex is more enjoyable. Embolization was a good decision for me.

So far, clomid and LongJack have both improved the way I feel. Still waiting for my tests as proof. I'll post results when I get 'em.

How about you?

I poste some of how I am progessing on another thread .... But to date I can say that my depression has really diminished, I am feeling more of my libido coming back, I am not focusing on the failures like I use to, but when I do fail it does make me over analyze, I still have tremors in my hands and weakness in my legs, sometimes they feel like they are gonna give out on me, I made an Appt. to see my GP on Tuesday, I do have more ambition than before the surgery, but still very content to be lazy, my dreams, OH Boy do I have some vivid dreams now, Before the surgery I was barely remembering them, now I can at least remember parts of them.
The worst thing about this is that I just hate not being who I was before .... I was a very sexual man with my other half .... Now it is a struggle to get into it, but I can say that once into the groove so to speak, I am pretty much like I was, it's just gettting over that hump, and I don't know if it is because of the fear of failure or the varicocele or the combination of both .... I have been dealing with this now since before Sept.

Are you on any anti depressants ? My Doc had put me on Welbutrin, but after the surgery I stopped taking it .... I am wondering if I should start again, if that could help in any way how I am feelng about all this ?

I really appreciate the support I've found in these forums .... I look forward to reading responses ....

TXathlete, sorry we got sidetracked. Just to recap for you... talk to doc about testing Estradiol E2 Ultrasensitive (at quest if possible)... if E2 high, talk to doc about an AI (eg. arimidex)... if E2 ok, talk to doc about SERM (eg. clomid). If SERM fails, talk to doc about HCG. And oh, don't forget about that thyroid.

That list cost about $3500

I doubt that only Medicare covers it.
As far as I can tell all is in ICD-9 codes.
ICD-9 ---> outright diagnose or just suspicion of a type of problem

On the bottom of list I posted my ICD-9 codes.

When I was working there was always variety of insurance plans, I would always pick one that cost most and covers least and gives least restrictions. Somehow there was always plenty of opportunities that made life easier going that way.

My daugher trying to save ($50) on insurance picked some s...t choice. Today with my wife they drove 40 miles to some boondogs, to see some jerk pediatric doctor for my grand-son.
Next time they will find out who it is going to be, for sure somebody else.
If that was about specialist, I would understand, no, routine checkup.
She is going to spend more on gas and tolls, and I am not even talking about inferior coverage.

Update

Been a while...still dealing with this mess. Thanks for the help back then...here is the update.

The Endo I saw put me on 100mg/day of Clomiphene Citrate for 5 days to determine if the problem was in my Pituitary or my Testes.

6th Day results:

So my LH and FSH both went up to High-Normal levels, but my Test only went to 317. I didn't know my Estradiol level before so I don't know how it was effected.

The Endo said that since the LH and FSH reacted positively to the Clomiphene that my Pituitary is fine, but that my Varicocele must be hindering the production of Testosterone. He said that since I am young, and varicoceles are progressive, that I should have the varicocelectomy done and then we can more effectively try to get my hormones on track.

So he has recommended me to one of the top varicocelectomy guys who I am seeing on April 16th.

I asked the Endo if I should continue taking Clomiphene, or some other SERM, since it raised my LH and FSH and at least raised my Test level a little bit...and he said there is no need until after the operation.

I realize that it won't be very effective until I take care of the varicocele, but I will at least feel better (less aches and pains, more energy, and better sex) until the operation, and I don't think taking a SERM until then will have any negative effects.

I bought some Nolva a while back because I knew my Test was low and Estrogen was high, but I never took any of it because I wasn't completely sure of the quality and I didn't want to cause any more problems. I don't know the rules about asking this, but I got it from a very "innovative" website...and I believe it is the site that I have seen recommended on here before. If anyone can attest to the quality of their products I would greatly appreciate it.

So here are my questions:

My initial appointment with the urologist is on April 16th. Can anyone who has had a varicocelectomy give me a timetable of how long after their doctor decided to have the operation, until they had it done? I am hoping to have it within 2 weeks of the initial visit, which would be by the end of April. Is that realistic?

After taking the 100mg of Clomiphene for 5 days, will my levels rebound to lower than before, or will they stay the same for a while? (asking because sex seems to be even more of a problem since I've been off the Clomid)

If I should take the Nolva, what dosage should I take? I've read that when it is used as a PCT that it is normally started at 40mg/day for one week and then 40/20/20 for the next 3 weeks. Since this wouldn't be for PCT should I use a different protocol? Would 20mg/day until the varicocelectomy (approximately 4 weeks) be an effective dosage?

I don't know how to "bump" things so more people can offer their knowledge, so if someone else could do that I would really appreciate it!

Okay .... I will try to give you as much info as I can on how my situation has gone ....

I too had a varicocele and it was a biggie .... 20 ligated on the Left Side - 7 ligated on the Right .... So I had bilateral varicocele ....

This process was started in Sept. 07 .... I had gone to one Urologist that was a Dunce (Said to wear a Jock Strap and come back in three months.) I went to another and he was concerned and ordered MRI and CAT scans to rule out any issues with my internals .... He was not concerned with my T levels in the slightest ....

I had the Sugery on Dec. 14 07 and my follow up is scheduled for April 3 08 .... The pain is gone mostly as well as the bag of worms is greatly reduced .... ED, Libido, Weakness, Depression, Memory, Concentration, Anxiety, Gumption, Sleep, and weight are all still present ....

Went to an Endo at the beginning of March and he said my T levels are fine at 293 (241-827), Free 10.25 (5.00-21.00) I still don't feel fine .... I am on Wellbutrin 150MG a day .... The depression is not as severe .... Sex is still not what it used to be ....

I started taking Nolva 20MG March 28 and my plan is to take it on M-W-F ....

I'm scared of taking it but when the Docs are so reluctant to even admit that you are low .... What should we do ?
We are left to medicate ourselves .... I didn't want to .... I had it in my drawer for almost a month before I finally said I had to try something else ....
I am watching all caffenine .... Basically have eliminated it from my diet .... I started being more careful of what I am eating as my blood pressure has risen, which I am sure is because of the low T levels ....

I hope all this helps man ....

I can relate. My DR. said my T level of 330 was fine and that I should just take 100mg of DHEA a day. When I had trouble trying to get him to order the correct bloodwork over and over I said "screw it" and made an appt. with Dr. John.
My well being is just too important to keep playing "grab-ass" anymore.

Becker, can you keep me updated on how the 20mg of Nolva on a M/W/F protocol works out for you? I am still trying to decide if I want to try something similar until I have the operation.

Can anyone else help me with these questions:

Come on guys...I know these must be pretty basic questions for some of you. Are none of you responding due to my reference to the "innovative" website or what? If so, can somebody just PM me with some info???

1. If my Test was 266...I took 100mg/day of Clomid for 5 days and it went up to 317...will it continue to hover around 317, will it go back to around 266, or will it drop to even lower than before???

2. Are the products (Nolva) from the "innovative" website considered to be safe and worthwhile???

3. If the Nolva is legit...what type of protocol should I use from now until the varicocelectomy (about 4 weeks)??? I am thinking something like 20mg/day or EOD (every other day), but I really don't know since all the info I have found on here about Nolva is in regards to PCT.

Please chime in with any words of wisdom...they will be GREATLY APPRECIATED!!!

I'm sorry that no one else seems to be reading your posts but me ....

I recently started the Nolvadex 20MG M-W-F .... I am only on my 3rd dose .... I can say that I do feel somewhat friskier .... And not as anxious ....

On your first question .... I would say that your level, as well as mine would drop back down .... It would most likely be over time and not all at once .... At least from what I have read is what I would think .... And what I am hoping ....

On your second question .... Value Pharmacueticals is pretty innovative ....

On your third question .... I would start as I have .... M-W-F and see how you feel ....

What surgery option did you choose ?
