Well 2 weeks ago i suddenly got violently ill out of nowhere. Went to the emergency room 6 times in the past week for shortness for shortness of breath and couldnt breath, and spent a week in hospital and doctors still dont know whats wrong...

I have diarrhoea, shortness of breath (comes and goes throughout the day), chest pains, muscle/weight loss, loss of appetite, bone/joint pain in my knees and pelvic bone, joints feel sharp (couldn't walk the other day), extreme fatigue (any exertion leaves me feeling ill), and i got these huge dark bags under my eyes, pulse when i wake up is 100 then later on its like 40-45, then sometimes when i get up out of my chair it starts racing and hair loss. I'm out of hospital now and still about to see a lot of specialists to figure all this out, its been a scary experience.

Blood work showed nothing wrong but i will post it when i get the chance, lung function test was excellent, muscle function was good but they said i was a bit weak in the hips (thighs and the surrounding joints), white cell count was normal so its not a virus or infection but my lymphocytes were elevated. CT scan of the chest showed residual thymic tissue in the lungs but they said its normal for someone of my age, also about to do a DEXA scan, 24 hour holter monitor (for the heart), and see an endocrinologist. Also had a endoscopy and colonoscopy (which they will redo because they couldnt see much first time in the colonoscopy) both which were normal except their was inflammation of my small intestine.

And last 2 days i could walk very well the joints in my knees feel odd and corroding and to the touch my right knee is painful, feels like my knees are going to fall apart.

I took anti-histamine Periactin and it relieved a good deal of the symptoms i mentioned up above besides the joint troubles and i'm using Voltaren tablets for the joints to reduce inflammation.

Also going to see immunologist because i think this might be an autoimmune disorder.

Any tips would be good nobody knows whats wrong and neithe do i, im really lost as to why this is happening its been a scary two weeks i thought i was going to die and for someone who's 19 this really is unexpected, GOD I WOULD JUST LIKE TO BE HEALTHY!!!.

Where did you lose weight? Whole body or here and there. How did the doctors find that you are weak in hips and thighs? How about your sleep? Can you sleep at a trecth for atleast 6-7 hours ?
Did you anything abnormal (apart from your routine work) two weeks back?

losing muscle weight but also weight loss all over, and even my joints feel thinner if that makes sense. I went to the emergency room and the doctor their did a muscle test on my upper and lower body because i was complaining of joint pain in my knees and left pelvic joint and she said i was weak in the hip/thigh region. Sleep yes i have been sleeping a lot more than usual last few days 14-16 hours then in the week before i was awake for 3 days straight and not tired. I just feel ill, like a feeling of malaise something isnt right in my body.

"Did you anything abnormal (apart from your routine work) two weeks back?"

no i didnt do anything abnormal then usual if thats what you mean...

I also have endocrine problems, low t, high e and cortisol so this could now finally be manifesting itself through my joints or it could be a immune problem i really really dont know...

Also any medication i take makes these symptoms worse even the medications ive used previously without any trouble like arimidex and thyroxine i experience chest pains and really bad shortness of breath if i take them now like some sort of allergy.

Also noticing i cannot tolerate dust and had to get anti-allergenic sheets and pillows and i keep everything ventilated now, i cant stand closed areas it feels like theirs very little oxygen.

About to see:

Lung specialist, endocrinologist and do a DEXA scan, immunologist, 24 hour holter monitor, gastrointestinal specialist to talk about what he found in my endoscopy and thats about it.

Its scary i cant walk very well today and this happened suddenly also in these past 2 days with the joint pain and odd feeling in the knees and my left pelvic bone.

and waiting on blood work for IgE a mast cell tryptase test which i requested be done because last time my IgE was 2000 two years ago 20 fold higher than normal.

I agree that many of your symptoms are consistent with an autoimmune disorder. Lupus and rheumatoid arthritis can note similar presenting symptoms, but at this point it's pure speculation until there is consistency with your clinical symptoms and blood work to support a diagnosis.

Did they do an ANA(Anti-nuclear Antibody), Sed Rate, Rheumatoid Factor, and Anti-DNA blood tests while you were in the hospital?

I would also run by your primary about seeing a Rheumatologist as well.

Edit: Are you experiencing and type of rashes, and if so where?

They didn't do any of those tests no.

rashes no... although my right knee is a bit red.

I will ask my GP to do those tests, thank you.

I almost amazed given your symptoms that they didn't include an autoimmune disorder in their differential diagnosis, or so it would seem anyway. If they had considered this they would have ordered similar tests as i've stated to at least rule it out.

I would definitely inquire about getting those tests done for sure. They can at least give some preliminary suspicion if found to be elevated.

Have you been tested for celiac (IGE gluten allergy)? I've known people who were very sick because of it. Causes intestinal inflammation.

Lattest problems that landed him in ER are probably due to too agressive candida treatment.

Celiac disease is technically an autoimmune disorder based on its clinical pathophysiology of gluten protein interaction with the microvilli in the small intestines causing antibody development against its own tissues. It's the body actually attacking its own intestinal tissue here though (not reacting to the presence of a foreign antigen specifically), and as a result the mistaken immune response causes the localized inflammatory tissue damage. It's commonly confused as an allergy however.

I agree that elevations in immoglobulins such as IgE suggest further need to explore the over-stimulated immune response. This includes looking into the other 4 immunoglobulin levels of IgM, IgG, IgD, and IgA. An extensive panel needs to be done here to futher determine the next course of action.

Thanks for pointing out that thread, i completely missed it. I'll have to read through it and catch up.

Was the candida resulted from corticosteroid treatment, or idopathic etiology (unknown cause)?

Candida is from an unknown cause.

I have tested:

COELIAC SEROLOGY

Gliadin IgA: 6 (0 - 20)
Gliadin IgG 3 (0 - 20)

So no its not coeliac or gluten intolerance, only food i know i am allergic to though to a very small degree is whole cows milk and strawberries.

IgE was tested 2 years ago

IgE - 1939

Common Grasses and Dust Mites both we're 4 on a scale of 1 to 4 to the degree of allergy to the substance.

Normal range is 100 and under.
(Atopic genesis highly probable)

Going today to my GP to get my latest IgE results. And i will ask him to test ANA, Sed rate, rheumatoid factor and anti-DNA antibodies.

Where do i check the 4 other immunoglobulin levels...? i assume this is best done with an immunologist.

Yes, check with the immunologist.

Metal i have to ask buddy, but have you been tested for any type of immunodeficency cause of symptomatology as well? ( IE. HIV, Cancer, autoimmune)

I say this because candida of unknown origin is highly suspicious of an immunodeficency disorder in general.

By NO MEANS is that what i believe is the issue, but it's an avenue that certainly needs to be ruled out.

No i havent been tested for any immunodeficencies
Again i'm suprised this wasn't a differential diagnosis ruled out to begin with...

This is really scary, i seem to be losing muscle mass by the day. And the other day my weight was 100kgs now 2 days later its 107kgs... and ive seemed to gain weight around my middle and still losing muscle mass.

Because this is all happening so suddenly i dont know what to do... im seeing a specialist tomorrow who did the colonoscopy/endoscopy and i will tell him about this, and maybe he can put me in hospital again if i convince him, because i was in their for 3 days and they let me go to see all these specialists as an outpatient but i cannot do this i have no energy or strength to live.

Any ideas of what i can say to him so he can put me in hospital again?

he even though he is a gastro specialist wrote that i have metabolic/endocrine and diabetic issues since my blood sugar was low in the glucose tolerance test

Can you just go in to the ER and skip the DR. See if they can give you the tests?

I hope you get it figured out soon so you can get better!

I would really voice your concern here and your need for peace of mind. I know how frustrating the medical field can be when you feel like you're lost in limbo.

I would suggest maybe admission to the hospital so all these specialist can get in to see you and get this resolved. I would push that point how much easier it is to get this accomplished this way. Good luck with all this bud, i certainly can understand how frustrating it can be.

Thanks for the support everyone. And no i cant be hospitalised unless im dieing its unlikely i will be hospitalised even then their is probably a waiting list :p

Ok so the gastroenterologist found i have a case of gastritis (inflammation of the stomach) though it isnt severe.

Can this cause any malabsorbtion issues and what are some good ways to reduce the acid in the stomach, i am taking sodium bicarbonate which apparently helps to keep your body more alkaline but other than that im not sure what else can help any ideas?

Also i am seeing new endocrinologist (hopefully she isnt a total idiot like all the others in the past who when they speak to me feel intimidated by my knowledge and dismiss any of my statements due to their ego, but she is young so their is hope) she also asked for me to do a DEXA bone density scan which i really think is a good idea since im having joint troubles likely to do with having low T all this time.

8am to test:

ACTH/Cortisol
LH/FSH
Testosterone
SHBG
TSH/ Free T4
GH
IGF-1

Then i will see her at 2pm, pretty basic but it should give me a insight of my levels at the moment... she is aware i have a slight case of gyno but i will manage that myself if she is oblivious to E control with the arimidex i have on script from my previous doctor.

Just out of curiosity do doctors ever prescribe things other than testosterone such as primobolan? I know a friend from my country of serbia who is on replacement their with primobolan and he said it quite common for it to be prescribed as androgen replacement in europe any ideas...

Heres some other info if anyone can add their knowledge:

CT Chest and Upper abdomen with contrast. (5th september)

The lungs are normally inflated and clear.Their is no interstitial lung disease and no infiltrate or evidence of bronchiectasis. No pleural or pericardial effusion.

There is a little residual thymic tissue in the anterior mediastinum, within normal limits for the patients age. No phadenopathy or other mediastinal mass.

The liver, spleen, bilary system and pancreas are normal. The adrenals are also normal. The right kidney is a little large (13.3cm) and the left kidney globally contracted (9.5cm) There are two renal arteries to the left kiddney but neither appears to contain a stenosis to account for the small renal size. The single renal artery to the right kidney is also normal in appearance.

Conclusion:

Clear chest, No mediastinal mass. Small left kidney.

Although this could also be my hashimotos hypothyroidism going into hyperthyroidism temporarily... when at the emergency room a week ago with all my symptoms they tested mt TSH at 3.7 and T4 at 19 the highest its ever been and ive been OFF thyroid meds for about 2 weeks and just doing the candida treatment.

Now my thyroid levels and TSH 4.5 and T4 16.8 without ANY treatment and before this my T4 was about 9. Range is 12-22 so this would certainly explain the symptoms... but i will get more insight into this issue when i do the new bloods in a weeks time and go from their.

Can anyone think of what this could possibly be... im seeing my GP tomorrow to ask him hes the only one i havent asked.

Basically symptoms are:

*Weight loss (muscle and fat)
*Then in 2 days my weight jumped up 6kgs now its going down again
*loss of appetite
*Less urination and thirst then normal (before i was very thirsty and urinating all the time)
*fatigue upon any type of exertion
* knee joints feel tender and when walking like they are made out of jelly, its becoming difficult to walk (happened suddenly last 3 days).
*High pulse in the morning and up and down throughout the day
*Occassionally freezing cold hands and feet
* Alternating periods of feeling alert and very tired
* shortness of breath (which seems to have subsided in the past 3 days)

Symptoms sound like: Carbon monoxide poisoning,Hypogonadism, and withdrawal from narcotics. Another possibility is an allergic reaction to detergents, household cleaners or some other substance that is being intentionally or unintentionally ingested. I would: re-locate for a day or two, try a one day fast, stop all supps and see how I felt.

Regards to gastritis (I know this is the least that concerns you), I've had/have gastritis but not severe. My doctor told me to not consume a whole lot of spicy food, such as jalepenos, hot sauce, salsas and etc. Supposily spicy ingredients is what causes it. I have lowered the consumption, and in result, I feel better.

I do hope and pray that you get answers fast but most of all I pray that you get better my brother.

hypogonadism yes. Allergic reaction to substances also likely i have very high IgE... and i am a supplement junkie and have always been using plenty of supplements all the time... mightve caused my gastritis, i will try to stop all supps and fast for a day.

Interesting you say that about relocating... I was just at my sisters place for a week and was quite unwell their and when i got home a friend of mine told me she cleaned out my room and vacuumed etc... and last few days being here the shortness of breath/chest pains went away as well as the slight dizziness and i feel a lot better.

Who knows maybe Cramming tones of supplements into me all the time is behind this, hopefully i can sort this out and my endocrine and immunological issues which are the main problems then resume a normal life and enjoy the things i love to do like bodybuilding and use minimal supps.

Thanks knowbull and Manny1010.

hahaha yer mightve been the devil.

Well i got my IgE results back:

IgE Total: 3810 IU/ml (0-100) Jesus! thats high!!!

Mast Cell Tryptase: 2.7 IU/ml <11.7

This is basically Hyper IgE Syndrome

Inherited Immunodeficiencies: Hyper IgE Syndrome
Definition
Hyper IgE syndrome is a heterogeneous group of disorders characterized by recurrent staphylococcal infections, unusual exzema-like skin rashes, severe lung infections that result in pneumatoceles (balloon-like lesions that may be filled with air or pus or scar tissue) and very high concentrations of serum IgE. Some patients have an autosomal dominant form of the disease; these patients have problems with their bones including recurrent fractures and scoliosis (curved back). Many patients with autosomal dominant hyper IgE syndrome fail to loose their baby teeth and have two sets of teeth simultaneously.
Clinical Features and Symptoms

*
recurrent staphylococcal and candidal infections
*
unusual exzema-like skin rashes
*
severe lung infections that result in pneumatoceles (balloon-like lesions that may be filled with air or pus or scar tissue)
*
very high concentrations of serum IgE

Some patients have:

*
problems with their bones including recurrent fractures and scoliosis (curved back)
*
fail to lose their baby teeth and have two sets of teeth simultaneously

Treatment Strategies

Most patients with hyper IgE syndrome are treated with chronic antibiotics to help protect them from staphylococcal infections. Good skin care is also important in patients with hyper IgE syndrome.
