Have heard of people getting the entire gland excised, which would seem to be the way to go. However, a lot of offices say on their websites they leave a small portion of the gland to prevent the nipple from being "dented in" in or concave?

Any opinions on the best way to go?

Seems like if you have muscle it might fill your chest out and you'd be less likely to get a concave nipple.

This. When I go to the doc I found in my area, I plan to be straight up with him and tell him that I want as much of the gland out as possible, since that will minimize the possibility of future gyno issues.
