34 yr Need Some Advice on TRT

34 yr old, 181 lbs, roughly 17% bod fat

Last year, I noticed I was feeling very lethargic, etc., so I had my blood levels tested. As expected, I had low T. So for about six months, I exercised regularly, switched to a plant based diet, took vitamins, lowered stress, and got a ton more sleep... Basically, I tried everything.

I got my blood tested today, and here are the results:
Total T: 330
Free T: 7.21
SHBG: 24.30
Free test: 2.16%
Bioavailable test: 194

So here is my question... I want to know what I am talking about when I visit my doctor later in the week, so I want some advice. My concern is that I want to have kids, so I don't want to shut my testes down and kill my sperm count. Is there anything I can do to get my test up AND remain fertile?

Any advice is much appreciated.

Dude.... Try the 35 & older forum. It's full of helpful info on low T & lots of people talking about TRT & stuff.
You'll want to seek out the master of testosterone IMO , thebigt
This guy knows his stuff.

Your T levels are waaaaay low! Your T level should be around 700-800. I have low T and mine was right about where yours was. My doc gives me shot of Test-Cyp, once a week at 100mg a dose. Within about two months my level was between 700-800 and that is my sweet spot. Energy is back, I'm putting on muscle again, and my sex drive is back! Got to get that right man, cause I'm telling you it will have a calming affect on you as well. My wife says I was very irritable before, but that is gone as well. Good luck sir!

OP, do NOT do this if you want to remain fertile. Test will make you infertile. It may be reversible in the future, but it will take awhile before you can produce viable sperm again. If you want kids sooner than later, do NOT do this. I am 31 and have been on trt for 1.5 years. I was first rx'd test from an endo but sought a second opinion because my wife and I want children. I decided to go to a specialist at the Cleveland Clinic and he began by telling me how sad it is that endos just throw young guys on test without warning them of the infertility issue. He put me on Clomid, 50mg EOD, which has raised my test from 190 to between 500-700 based on 4 f/u tests over last 1.5 years (that is normal range for my age). More importantly, I am still fertile.

Talk to your doc about Clomid.

Now with having kids, he's probably right. Explore the options with your doctor and do what's best for you. For me, my wife and I have two kids and are done with making more. But your levels are too low man. Again, good luck.

Lucky bastard...my wife is dying to pop one out! Aside from the fertility issue, test cyp is what would likely be prescribed and that is what I'll be on once we're done making babies

i have heard good things about clomid being used to restart test production in borderline cases, your case shows dramatic increase-congrats!!!

for long term use though i would still go with testosterone.....who knows, after a good amount of time on clomid treatment your test level may remain higher!!!

good luck with having that baby!!!

BRUstrong,

Thanks for the reply... What do you think about running a bit of clomid + hcg along with the cyp? In your non-medical opinion, you think this alleviates some of the concern?

Thanks man!! Yea, luckily the Clomid has worked well. Studies show safety in males for up to 3 years, but I believe there may be some newer studies showing safety up to 5 years. Regardless, test cyp is still the better way to go, and my doc told me he will be switching me over once fertility is not an issue.

Got dealt a sh*tty hand because my wife has premature ovarian failure at age 29 so we already have 2 unsuccessful IVF attempts under our belt. Going to try a third round here in a few weeks then we'll probably have to look into a donor egg. Anyway, it'll happen one day!

Not sure about clomid but have heard that hcg may help. Honestly I have no idea for certain, but some of the other, more knowledgeable guys on the forum may have some insight

he really needs to post in 35 and older section, some really knowledgeable guys there.....my wife and i are both in our 50's so i have never had any reason to research trt and fertility, my main concern has just been lack of sperm production for increased pleasure and satisfaction.

off topic, but i am really pulling for you and your wife to conceive...i see so many women having babies they don't really want, or just for their own selfish interests, be nice to see couples like you and your wife having more babies!!! my best wishes on your good luck!!!

So I went ahead and took a 200mg shot of cyp on Thursday. This Tuesday and Wednesday I'll be taking 500iu's of hcg. I've got some arimidex on hand in case things go haywire.

Am I missing something here? Im getting most of my dosing/ info from guys that have been at this for years, but since Im not trying to look like a supehero , I just want to make sure Im not too high/ low on anything. I just want to feel better, and maybe get a little kick in the gym. I don't want to die over a six pack lol.

Feel free to beat me up over this if you want... You guys have been a great resource, so I owe it to you.

All I take is 100mg/week and my levels are between 700-800. For me, anymore than that and my estradiol gets too high. I don't need any AI's otherwise. You're going to feel better, just monitor your blood and you'll find what works. Good luck sir!

What were your numbers when you started? Do you feel a significant difference in terms of mood, training, etc?

My test level was 370 when I started. I'm 43 years old, and I assumed my lethargy I was feeling, my need for Viagra, and me being a tad irritable (according to my wife) was on account of turning 40.
I went to my doc for help to lose weight cause no matter how much I lifted, did cardio, and stayed spot on my diet I couldn't lose any weight. She checked my blood to start, before any plan was hatched to get me healthy, and there it was. This was December of 2013, I was 276 lbs at 5'11"! She started me on the Test Cyp and told me to continue my workouts but to up my protein.
So today, I'm 215lbs (my goal was 200lbs), but that has changed over this year because that "extra" weight is muscle my friend. My waist was 42", now it's 35". My wife loves the new calm and fit me, and so do I. Oh yeah, no more Viagra for me buddy! Obviously, it has changed my life and I tell everybody in my gym just get your levels checked cause you just don't know.

Look into a clomid protocol. Then bring it up with your doctor.

Edit: nevermind it seems you have already went the TRT route.

I started feeling the Test kick at week 3. I noticed waking up in the mornings with energy and I also noticed I didn't get as agitated at other "bad"drivers on the road. Lol. It's funny, but I did realize that it didn't bother me one day while driving. From there it just kept getting better. You took twice as much as I do, so you will probably feel a bigger kick at that third week. The Test will take a few weeks to build and then it's about maintenance when you and your doc decide you like where you're at. You'll get there faster with your dosage. It took me about 2 1/2 months to hit the 750 level at 100mg/week.

I would look intk a proper PCT and HCG use. Pinning Test is the wrong approach.

If you really want kids... knock your girlfriend up first and then worry about TRT
