Just look at your doctor's limitations, he have to stay with these limitations, unless you are on some kind of research program.
These are his guidelines;
GUIDELINE TITLE
Testosterone therapy in adult men with androgen deficiency syndromes: an Endocrine Society clinical practice guideline.
http://www.guideline.gov/summary/summary.aspx?doc_id=9420&nbr=5041
1. Infrequent, and mostly too small amount of testosterone
2. No mention of estrogens
3. tunel vision, there are other systems in the body that usually need attention.

My case, same hospital different doctor.
I have a (removed) cancer, carcinoma in left thigh.
When I tried to discuss my treatment, he wipped out a chart.
Told me where I am on that chart, no more discussion.
Everything done in extremely polite (and efficient) manner.
They have database of about 10000 cases, do statistical analysis, very impressive.
I am still dancing, he saved my leg, no complains.

Androgen deficiency syndrome is cutting edge.
Actually I do not even like to say androgen deficiency,
anti-aging more like it.
Unfortunately, I do not like what I hear about anti-aging clinics either.
From the few posts of people who went there:
no cutting edge information
cookie cutter approach
minimal testing
way out of line prices for so little investigation.
Hovewer anti-aging as a general approach is more appealing to me.
At the moment
Dr John
Dr Shippen
Dr Marianco
are the cat's meow.
