VARICOCELE...can it effect testosterone?

  1. Varicocele Emobolization complete about 15 hours ago.

    So far 0 pain, they gave me 4 ibuprofen post op which I didn't need.

    Overall it was a painless experience, but it does take ALL day. I checked in a 11 am, and didn't get to go home until 7 pm. After check in, you hit pre-op, then to meet with the Doc. He explained exactly what he was going to do, and what he was going to look for. He answered all of my questions and was very detailed overall. Of course I had to ask when I could get back to lifting and he said about a week. But then said that doesn't mean 100% in a week, he wants me to ease into it a bit. He also described how our body is great at adapting and may find other routs to cause the varicocele again, which if happened I can do the micro surgery.

    They then brought me into the surgery room and put me on the table. It was supposed to be a "Twilight surgery" with fentanyl and some form of anti anxiety med, and zophran. However once the zophran and fentanyl went in my agitation level spiked, the room started slowing down and I was fighting it. Nurse asked "You bp went up, how are you doing?" I said my anxiety is through the roof. She said ok Ill do another dose....1hr later I heard them asking me to take a breath and hold it while they ran contrast dye. Surgery was over, and I slept through it.

    I asked what type of material they used for the coils and they said nitinol (Nickel/titanium.) I asked how many coils it took, and they said "10 at about $90/piece that was an expensive vein!"

    They showed me the imaging of what they did and it looked similar to the online pictures I had looked up. They also said they checked the right side but everything there was functioning fine.

    I was then brought up to post op where I had to lie flat for an hour, 45% for an hour or so, then upright, then walk around about 3 hrs total.

    The varicocele used to take up the entire left side of my scrotum and felt VERY full. Today its about 75% reduced and the veins are not full at all.

    That's about all I can think of, I will update with progress.

    *Update: 24 hrs in there is finally a little bit of pain on my right hip, which was the catheter entry point opposite side of the coils. I would rate the pain somewhere between a bruise, and a lightly pulled muscle. Nothing crazy but figured I would update for anyone considering the procedure.

    *Day 4 update:
    Varicocle is down 85% of what it used to be. I read it can take up to 6 months for it to go away, but amazing progress so far.

    Pain on the right side has been gone for a day or so, replaced by intermittent pain in the vein on my left side. Again nothing Ibuprofen cant handle.

    Sleep has been different since the surgery, I remember my dreams EVERY time I fall asleep now, and seem to hop right out of bed. I always thought I slept well in the past, but was always sluggish waking up and only would occasionally remember dreams.

    Morning wood: Every day for the last 3 days, compared to once every 2 weeks before. Possibly a sign of test levels normalizing.

    Increased sensitivity: I was not expecting this but it makes sense. With the grade 3 varicocele, my left nut was essentially locked into place when I stood up for the last 14+ years. Now that its free hanging it is a lot more sensitive, and something I will have to account for.

    Thats all I can think of for now, more updates to follow

  2. Do you guys think it'd be pointless to get the emobolization done if someone's already been on TRT for almost 10yrs, without hCG the majority of that time?

  5. It seems that this is the goat thread on the internet about this. I created an account just to post here..

    I've been aware of my varicocele for quite some time but I was too scared to get it checked out, and eventually forgot about it, but recently after I was doing heavy lifting I became conscious of my problem again.

    Two weeks ago after I noticed it again, it started to be painful and I've never been more scared after reading about how it can lower your test levels, because I've always been depressed with no confidence and recently I have been having problems getting hard I believe.. idk.

    Anyway.. I just saw a doctor for varicocele and got diagnosed with that, but also with epididymitis (wtf). Now I'm prescribed antibiotics but idk if I should take them..

    I'm really scared.

    I'm also seeing a urologist in ten days.

    I just want this to end so I can have a normal life.

    the spermatic cord feels a little different but pretty similar to the other side.

    the testicle did grow back a little bit.

    the area of incision is swollen a little bit.

    i do have occasional pain but i have been working out for 4 months straight with heavy lifting and running. i feel good.

  9. Well I promised myself to write my experience of having my varicocele operated on this thread, and here I am adding my two cents. Currently I am 28 years old. I developed varicocele around mid of 2014 and it wasn't until the 25 of May, 2015 that I was diagnosed with a varicocele after an ultrasound. I did in that time experience a decline in strength of my erections, a decrease in size of testicles, especially the left one which felt softer too. The scrotum area felt hotter. I met five doctors and made an impression of each. I liked only one, a younger doctor around mid 40's who had done over a thousand of such operations. He recommended the surgery though he said my balls looked fine and they are not small at all. Well I chickened out and thought to discard any option of surgery. More than two years I remained in a dilemma, an extreme mental struggle. I had decent morning and voluntary erections, but the frequency, ease to attain one did decline. The strength of the erection when I was fully aroused was the same more or less. The refractory period seemed to have increased too. Mentally I was draining. The constant thought of having my testicles suffer progressive damage and shrinkage was killing me. I felt lesser of a man. The varicocele ran on my mind every second of the day. My mood was always unpleasant, my attitude towards others was indifferent, and the anxiety was paramount. Maybe I made varicocele a problem bigger than it was, none-the-less it was there. I never told anyone what I had. Every doctor told me to get a semen analysis which I never did till date. I did however got a Total Testosterone test, which came out as low as 360 ng/dL (Lab range: 160-729). In my defense I took the test in the evening at 18:36 on a day when I woke up at 10:00. I didn't know you are supposed to do it in the morning at that time. I also did another detailed ultrasound later on where they measure the diameter of the dilated veins. The maximum diameter on the left was measured 3.9 mm and on the right side it was 3.6 mm. The normal diameter should be around 1.4 - 1.6 mm, so mine were pretty dilated. So for more than two long years, I tried fooling my brain into thinking I was fine when I was not. I couldn't concentrate on anything and constant had to check my balls for some odd reason. It wasn't only me who was effected, but people around me were too I guess. My mom had no clue of why I was acting so wierd and indifferent. So I only told my mother of it, we all do tend to confide in them for they keep the secrets well guarded. She has been the only support for me through all my life and telling her did provide me some relief. She said to stand by me on whatever decision I would take. But my mind kept fluctuating if to have it operated or just leave it as I could get an erection just fine provided if I am in the right frame of mind. I also noticed that gradually my urine flow got weaker, and interrupted like it would start and stop, possibly directing to a problem with my prostrate. I never had any pain because of the varicocele, a reason for me not having the surgery earlier. I surfed the net a lot, read so many forums, experiences, articles, and reports. I read them all, nothing new left on the net to read. I read this particular thread several times, compared notes and experiences. Finally, just recently I decided to have this surgery, get rid of my varicocele, halt any further testicular atrophy and damage, and move on with my life. I went to the same doctor and told him to operate me. Well he remembered me from our last meeting surprisingly. I asked for the earliest possible date of his availability, paid the advance right away and booked the surgery. I had the surgery on 19 Oct 2017, three weeks back.

    OPERATION DETAILS:
    Proceedure: Bilateral Micro-Doppler Varicocelectomy
    Incisions: Location; One inch from either side of the penile base towards the groin (Not visible at all). Incision Length; 1 inch
    Testicular and Vas defrens Arteries: Preserved
    Veins: Veins > 1.6mm ligated; On site, RT Vein (max = 4.2 mm) > LT Veins (max = 3.8 mm)
    Ligation Method: Tied at two ends and cut from the middle
    Operation Time: LT = 38 mins , RT = 25 mins
    Anesthesia: Spinal Anesthesia, Catheter used to urinate for apprx. 3 hours post surgery

    No findings of an internal testicular varicocele (don't know what that is). The doctor said I made a very good decision of having them operated as the varicocele was larger than the reports and the diagnosis. The only complication I had post surgery was that on the third/fourth morning, one of the dorsal veins in the left side of my penis got swollen. I believe it was due to the jockstrap that I was wearing which caused my morning erection to be bent downwards thus causing some sort of inflammation or irritation. The jockstrap helped a lot to provide support in the first week or two but remove them before going to sleep. Make sure to ice regularly to reduce swelling. Anyways, I paid the doctor a visit and found out that the swollen penile vein is called Edema. He prescribed a pill for it along with other antibiotics and medicines I was taking post surgery. Testicular torsion is not possible as the gubernacular veins prevent from happening such. He did not ligate the gubernacular veins as he said they are required to drain the testicles. Post-op swelling fades away completely after 3 months. Theoretically I can't play sports for 1 month but he advised to avoid sports for 3 months. The stitches were self-absorbent but I got them removed on the 10th day as advised. I took the first shower on the 13th day post-op. Follow your doctor's instructions post surgery. Next ultrasound should be done after three months of operation. Right now my swelling has reduced considerably. I was able to walk fine two days post-op. You will feel pain right after surgery once the anesthesia wears off, but the painkillers would help. The pain is not too bad, however there will be some pain on the first day but nothing that you can't handle. The testicles do feel cooler to touch and have grown in size, though the doctor said I should not expect my testicle size to increase. Libido is better than before. Erections are the same, the ease to attain one might have improved but the strength feel the same. Doctor said that a varicocelectomy would have no effect on your erections, and that the erections could possibly improve but they can't get worse. The reason being that a varicocele does not damage the Leydig cells which are responsible for producing testosterone as those cells are very resistant and enduring. The doctor did say that premature ejaculation usually improves after such procedure. Most importantly my mental stress has gone as I don't have to worry about having a varicocele anymore. Anxiety, mood swings and depression has gone too. I don't have any hormonal profile to see the changes but all in all I feel way better mentally. Like many others I too noticed that I remember my dreams almost every time I wake up. Just last night I saw a sexually very graphic dream and woke up to a very strong erection that took about 5 mins to fade. I believe this has to the with the mental state one has while sleeping. I had decent morning woods prior to the surgery too so can't say if that was an improvement. But the dreams are definately more vivid now. Before the operation I would be in a flight-or-fight state and anxiety while going to sleep, as a result of which I would wake up extremely tired and fatigued. Now I feel more active and rested after sleep. The urination problem has improved and the urine flow is more continuous now, maybe due to the 2 weeks antibiotic course I did.

    My advise to anyone considering surgery is to have it done. If its causing pain, testicular atrophy, higher scrotal temperature, mental fatigue and depression, you should consider having the surgery. Its a simple procedure with minimal risks. I am happier now knowing that I don't have a varicocele than I was before. I thank everyone for contributing to this forum and thread, and to all those people kind enough to respond me on PMs. Find a good urologist who listens to your problems, gives you enough time, doesn't rush and is an experienced male infertility specialist. Do not fear the operation like I did, of having my testicular artery cut, as the Micro-Doppler used ensures the identification and preservation of the artery. Stop drugs (as they act as vasodilators), masturbation (especially edging) and anything that could be harmful. Or at least balance everything so it doesn't effect you. Eat healthy, keep hydrated and solve every problem that comes your way. I don't present any thing as facts here, its all my personal opinions based on my personal experience only. I have tried to provide as much information as I could so that some one might benefit from it like I did from many of you here. God Bless!

I believe the answer to your question is that once the veins die (this happens in the first 24 hrs) then its just a matter of time for them to be absorbed by the body. They do not stay there forever. In fact, unless I am mistaken they could be absorbed within a couple of months.

    Did you have an hormonal changes as a result of the surgery? I noticed changes right away. The very first night I had an insanely vivid sex dream and have had constant erections since the surgery. I haven't worked out yet so I don't know how this will manifest in the gym. I also have not taken new blood tests yet to check my T levels and see if they have gone up as a result of the surgery, though i'm sure they have given my personal experience.
